The APP supports timely access to specialty care, continuity of treatment, and evidence-based infection management.Responsibilities • Perform consultations and follow-up to manage patients with infectious diseases in both inpatient and outpatient settings. • Participate in hospital consultations for sepsis, endocarditis, osteomyelitis, and other complex infections. • Manage outpatient cases requiring long-term antimicrobial therapy and monitor for treatment response and complications. • Ordered and interpreted labs, imaging, and microbiology studies to guide diagnosis and care plans. • Educate patients and families on infection prevention, antimicrobial regimens, and follow-up care. • Collaborate with physicians, hospitalists, pharmacists, and care management teams. • Document clearly and promptly in the electronic health record including consult notes and outpatient follow-ups. • Support antimicrobial stewardship and infection prevention efforts. • Demonstrate professionalism, teamwork, and commitment to high-quality infection related care.
