Records observations, treatments, and other pertinent information. Communicate with Medical Director, Attending Physician, branch leadership and IDG regarding findings from the visits and any measures to alleviate symptoms. Provides physical and emotional care to support the patient and family in period of crisis. Ability to triage consults by patient acuity. Performs bedside palliative care with patient/family education Ensures that all activities performed align with the vision of Elara Caring’s board of directors, executive team, and the leadership of the Hospice team. Prevents unwanted hospitalizations by meeting patient and family needs in the home. Prepares and maintains accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided. Compliance with applicable laws and regulations with respect to Collaborative Agreements. Completes accurate billing and coding activities for all patient encounters according to CMS guidelines and initiates reimbursement for medically necessary visits in accordance with applicable law and regulation. Provides training and continuing education for staff. Assists in development of clinical practice guidelines/standards in support of quality palliative care. Adheres to the practice of confidentiality regarding patients, families, staff and the organization. Collaborates/Communicates with Medical Director, attending physician, branch leadership and IDG. Performs other duties/projects as assigned. What is Required? Master’s degree in nursing and certification through a national Nurse Practitioner Licensing board. 1+ years of clinical nursing experience 1+ year post-graduate clinical nursing experience in the hospice and palliative care field preferred Prior experience in Family Medicine, Geriatrics and psychiatry preferred. Current licensure in the state of practice as a Nurse Practitioner and maintain such licensure in accordance with applicable laws and regulations. The Nurse Practitioner (NP) provider is one of a group of MDs and NPs who staff palliative care consultation services that function as programs of MJHS Hospice and Palliative Care. The NP Provider may work in a nursing home, hospital or community environment. Specialist skills in symptom management, communication including goal setting discussions, and hospice eligibility review, and may work independently or as part of a treatment team. Initial patient encounters are in response to consultation requests, and the initial consultation may be followed by one or more visits, as appropriate to the referral questions and status of the patient and family. The care plan advanced by the NP Provider aims to prevent or mitigate illness burden for the patient and family. Each encounter comports with best practices of specialist palliative care, including history-taking, appropriate examination, formulation and care planning, treatment recommendation or implementation, communication, and documentation. Other responsibilities include developing quality initiatives, educating staff, and supporting hospice operations through participation in the provider on call rotation and performance of face to face encounters as needed. Qualifications • Master's or doctoral degree in nursing • Current NP License within the State Responsibilities • The Nurse Practitioner (NP) is an experienced licensed professional who works in collaboration with the Hospice Medical Director/Physician to deliver hospice and/ or palliative care to patients and families • Performs prescribed medical treatments, including patient examination, diagnoses, medication prescription, pain management and symptom control; conducts assessments and evaluations, and provides education and supportive care to hospice teams, patients and caregivers • Performs comprehensive patient examinations, evaluations, assessments and diagnosis • Assesses for signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action to control and manage patient symptoms and needs • Makes adjustments to the already established plan of care and appropriate services, provides appropriate follow- up to all patients • Demonstrates flexibility in scheduling, and readily accepts new and emergency assignments as requested • Performs timely face to face assessments with patients • Demonstrates positive interpersonal relation in communicating with physicians and co-workers to promote quality patient care • Completes all documentation in an accurate, legible and timely manner • Provides quality patient care consistent with standards of nursing practices • Complies with hospice policies and procedures, laws and regulations and standards of practice This position will cover Sonoma, Marin, and Napa counties.
