The specialty NP is generally service/physician aligned, managing a distinct patient population. The NP may be required to perform some coordination of outpatient and inpatient care and facilitate discharges/outpatient follow up. Position Hours PRN (As Needed) Department Summary: The Newborn Advanced Practice Provider (APP) department is dedicated to providing comprehensive and specialized care to newborns. This team consists of highly trained Advanced Practice Providers (APPs). The department is integral to ensuring that newborns receive high-quality care during their admission to the hospital. Our providers perform detailed physical examinations and health assessments of newborns, diagnose and manage acute conditions in newborns, develop and implement individualized care plans and conduct routine screenings and developmental assessments. Our APPs work closely with attending providers and residents, pediatricians, nurses, and other healthcare professionals to deliver coordinated care. Our APPs participate in multidisciplinary team meetings and case discussions to ensure comprehensive patient management and facilitate smooth transitions of care from the hospital to home or other care settings. Pediatric Nurse Practitioner - Primary Care or Acute Care (CPNP-PC or CPNP-AC), Neonatal Nurse Practitioner (NNP-BC) or Physician Assistant (PA) with a focus in neonatology or pediatrics. Active and unrestricted state licensure BLS, PALS and NRP certification KEY RESPONSIBILITIES • Assessment of Health Status • Performs and documents in the medical record a complete history and physical examination for acute and complex chronically ill patients • Orders and collects data using appropriate assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated. • The nurse practitioner may serve as the first responder for both nursing and house staff in the event of patient status changes, orders, and in situations requiring procedural and resuscitative interventions. • Assesses for risks associated with the care of the acute and complex chronically ill patient including medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions and diagnostic procedures. • Coordinates daily clinic schedule with the attending physician team, presents patient report, collaborates in plan of care, and follows assigned patient population. • Diagnosis • Manages diagnostic tests through ordering, interpretation, performance, and supervision. • Formulates differential diagnoses by priority. • Diagnoses complications and orders appropriate interventions. • Formulates Plan of Care • Identifies expected outcomes from diagnosis, and formulates and documents a plan of care to address complex acute and chronic health care needs. • Utilizes evidenced based practice guidelines or protocols in an individualized, dynamic plan of care that can be applied across the continuum. • Implements and modifies plan of care • Prescribes diagnostic strategies and therapeutic interventions both pharmacologic, non-pharmacologic, and surgical, needed to achieve expected outcomes. • Manages further diagnostic tests through ordering, interpretation, performance, and supervision. • May perform advanced procedures consistent with privileges and competency validation, including minor suturing, complex wound management, line and tube placements and removals • Communication and Collaboration • Maintains ongoing communication and collaboration with interdisciplinary health care team. • Facilitates and communicates with patient, family and staff to promote continuity of care across the outpatient continuum of care. • Documentation • Documents/dictates key components of patient's progress via daily progress note, transfer, and discharge summary, and/or clinic note where applicable. Summary In collaboration with a physician, provides comprehensive assessment of patient’s health status, evaluates assessment data, and makes treatment decisions. Belmont (Charlotte Metro Region), North Carolina Responsibilities We have an excellent opportunity for an experienced full-time neonatal nurse practitioner, to play a leadership role in a brand new, 10 bed LDRP Level-I Nursery, at CaroMont Regional Medical Center-Belmont which is scheduled to open late 2024. About the Practice This Level 1 unit will have an anticipated 600 deliveries a year An average daily census of about 5 newborns NNPs will be responsible for daily rounds on newborns, attending deliveries when requested and will take call from home Practitioners would also stabilize, manage and facilitate transport of newborns to adjacent NICUs in Charlotte or Gastonia for newborns needing higher level care The Belmont team will consist of 4 NNPs with the sister LEVEL III NICU staffed by 2 neonatologists and 5-6 NNPs who provide care at C aroMont Regional Medical Center- Gastonia , which is only 7 miles away Opportunities to do supplemental PRN shifts at the level III NICU in Gastonia may be possible; but would not be required, as this position is for the Level 1 nursery. Per diem opportunity Job Summary Wentworth-Douglass Hospital is a community hospital located in Dover, New Hampshire, about 70 miles north of Boston and is a part of Mass General Brigham network. WDH Women & Children’s Center currently has over 1700 deliveries per year and offers complete family-focused care, making it the delivery destination for the New Hampshire Seacoast. The centerpiece of the WDH Newborn Program is a 12-bed Level IIB Special Care Nursery, offering care to infants born at 32 weeks of gestation and higher. WDH’s close medical and nursing collaboration with the Mass General Brigham Academic Medical Centers ensures the most advanced approach to infant care. The qualified candidate will be joining a dynamic group of advanced practice practitioners and physicians, led by a neonatologist medical director. Newborn and Pediatric services at WDH are supported by MGH pediatric subspecialties, including pediatric cardiology, pulmonary, endocrinology and gastroenterology. Newborn Nursery The provider will manage infants in the Newborn Nursery as a primary provider in collaboration with supervising physician. This involves daily rounding, daily comprehensive assessments, review of patient data, and development of plans for all infants. Diagnoses are varied and include those infants who are experiencing delayed transition, mild respiratory distress, hypoglycemic infants, late preterm infants, convalescing preterm infants, and infants born to mothers with substance use disorder. Responsible for daily care, coordinating and planning discharge home, supporting and updating families, and conducting family meetings. Qualifications PRINCIPAL DUTIES AND RESPONSIBILITIES Manages the health care of patients within Newborn Services through advanced health assessment, diagnosis, intervention, and evaluation in a collaborative practice agreement with a physician. Develops treatment plans, orders and interprets laboratory and diagnostic tests, treats patients with acute problems including illnesses and minor injuries following a predetermined protocol in a collaborative practice agreement with a physician. Coordinates the care of select patient populations to ensure promotion of health and wellness through education of patients and families, coordination of services and placing referrals. Coordinates patient transfers and admissions within Newborn Services. Participates in professional development and lifelong learning, including participation in resident medical education and training.
