This position combines remote clinical supervision of a team of Registered Nurses (50%) with in-field patient care (50%). This is not a traditional NP role. This position is designed for a clinician who excels in advanced wound care, thrives in team leadership, and is passionate about elevating care delivery in the home setting through mentorship, oversight, and hands-on collaboration. The NP will report directly to the State Medical Director and serve as the primary clinical resource, supervisor, and partner to the RN field team. Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers. Must be comfortable staging pressure injuries according to the NPIAP Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc. Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices Strong preference to having wound certification or be open to getting certified Experience working in a long-term care setting. Ability to work with an EMR.
