What You’ll Do:

As a Certified NP, you will be a key clinical provider in a skilled nursing facility, diagnosing, treating, and guiding patients to better health. No two days are the same!


Your daily responsibilities include:

  • Performing physical exams and reviewing medical histories.
  • Ordering and interpreting diagnostic tests (labs, imaging, etc.).
  • Diagnosing and managing acute and chronic conditions.

Provide Primary Care services to members of the South Central School Trust and their dependents and also provide Transitional Care visits to patients being discharged from JMH. This position will be housed at the Immediate Care/Occupational Health Center. Examine and treat patients under supervision of physician within the appropriate scope of practice, including, but not limited to: performing telephone screenings of patients referred from physician offices prior to appointment being made, obtaining patient histories and performing physical examinations, ordering and/or performing diagnostic and therapeutic procedures, formulating a diagnosis, developing and implementing a treatment plan, monitoring the effectiveness of therapeutic interventions, offering counseling and education to meet patient needs, analysis of vital signs and tests as well as exam findings and making appropriate referrals.
