Position Summary The PRN Nurse Practitioner provides services including assessments, clinical evaluations, medication management, patient education, and collaboration with the interdisciplinary team. This role supports home health patients with chronic conditions, acute changes, post-hospitalization needs, and ongoing follow-up care.
