Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans Modify patient treatment plans as indicated by patients' responses and conditions Monitor all aspects of patient care, including diet and physical activity Assess the needs of individuals, families, or communities, including assessment of individuals' home or work environments, to identify potential health or safety problems Perform other duties as assigned MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES) Associate degree required; Graduate of an accredited state-approved, Bachelor’s Degree from the school of nursing preferred Current and valid California Licensed Vocational Nurse license One year of LPN experience, Current CPR certification (required), Driver’s License, and Car Insurance. Complete all charting, changes in condition and/or new orders or treatments received for specific resident in their charts. Document schedule or cancelled doctors’ appointments in residents’ chart. (inform transportation of changes) For OOP residents please fill out appropriate form, and head to toe assessment completed. Complete incident report if any occur during shift (write incident on 24 hour report, and submit paperwork to unit manager or slide under DON’s door). Complete lab inquiries and noted on lab log for any new orders or any labs received (log them in).
