The L.P.N. delivers care to individuals in a community setting utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation. This person will interact with individuals, significant others, and health team members while maintaining the standards of professional nursing. The Director of Nursing leads a team including RNs and LPNs for an assigned region to care for individuals with intellectual and developmental disabilities. This person will support teams with day-to-day operations and challenges to ensure the medical and healthcare needs of the individuals we support are met. The Nurse Practitioner collaborates closely with the Registered Nurse (RN), Psychiatrist, and other team members to ensure comprehensive assessments, effective care coordination, and consistent psychiatric support for youth experiencing target behaviors. This role is vital in addressing medical and psychiatric needs, reducing service delivery gaps, and supporting families. Providing face to face supports for 10 kids. Pay Rate:$85.00/ per hour Responsibilities Conduct comprehensive medical assessments to identify potential conditions contributing to target behaviors Collaborate with the RN and Psychiatrist to integrate findings into care plans.
