POSITION SUMMARY Our Resident Care Coordinator serves as a liaison between residents, their families, and the care team, promoting a supportive and safe living environment. They support the Health and Wellness Director by assisting in scheduling, supervising staff, and ensuring that systems are in place to provide quality assisted living services in all resident care areas. Duties include problem-solving resident concerns and coordinating care with the team and external providers. Additionally, they help with medication administration and direct resident care when necessary. KEY RESPONSIBILITIES Schedule and assign caregiving and medication tech shifts to ensure adequate coverage and compliance with staffing guidelines and department allowances. Participate in the recruitment and interview process for new care team members. Supervise, train, and support care team members, promoting teamwork and adherence to care standards. Supervise the medication room and orders, collaborating with medication technicians and caregivers. Maintain accurate and up-to-date resident records, including care plans, incident reports, and medical documentation. Coordinate care planning with home health agencies on-site, working within community and company policies. Coordinate admissions with the sales team, including supervising move-ins. Complete family/resident admission interviews. Facilitate effective communication among residents, their families, and healthcare providers to ensure seamless care delivery. Address residents’ special needs, requests, and complaints. Assist with medication administration and provide direct care to residents as needed. Understand the community's care regulations, as well as state-specific regulations. Requirements CANDIDATE QUALIFICATIONS Education and Certifications: High School Diploma or equivalent. Must have a valid state medication administration license (a Certified Caregiver) per state requirements. An LPN/LVN nursing license is a plus. Current First Aid and CPR license or ability to obtain. Experience, Competencies, and Skills: At least two (2) years of experience administering medications and providing care in a professional caregiving setting is required. At least one (1) year of relevant supervisory experience, including staff scheduling, is preferred. Experience with memory care is preferred. Excellent attention to detail and high integrity. Excellent communication skills and teamwork mindset. Strong leadership and team management skills. In collaboration with the Health and Wellness Director, this role provides high-quality care through health assessments, medication management, and coordination with residents, families, and the care team. Responsibilities include monitoring resident health, addressing changes in condition, staff training and supervision, and promoting a safe and supportive environment that aligns with our mission to provide exceptional care. KEY RESPONSIBILITIES Perform resident assessments and reassessments according to policies, monitoring any changes in condition. Communicate with the Health and Wellness Director about resident needs to ensure consistent quality of services. When necessary, participate in hands-on care, medication administration, and specific nursing tasks. Schedule and assign caregiving and med tech shifts to ensure adequate coverage and compliance with staffing guidelines and department allowances. Supervise, train, and support care team members, promoting teamwork and adherence to care standards. Assist in the transcription of physicians' orders and the review of new physicians' orders. Maintain accurate and up-to-date resident records, including care plans, incident reports, and medical documentation. Coordinate care planning with home health agencies on-site, working within community and company policies. Address residents’ special needs, requests, and complaints. Coordinate admissions with the sales team, including supervising move-ins. Understand the community's care regulations along with state-specific regulations. Requirements CANDIDATE QUALIFICATIONS Education and Certifications: A valid, incumbent nursing license (RN) is required. High School Diploma or equivalent. An associate degree or higher in nursing is preferred, or a combination of education and experience. Additional coursework in management is an asset. Current First Aid and CPR license or ability to obtain. Experience, Competencies, and Skills: At least four (4) years working in geriatric nursing, including experience with medication administration, delivery systems, and pharmacies. At least one (1) year of relevant supervisory experience, including staff scheduling and management, is preferred. Experience with memory care is a plus.
