Job Responsibilities:

Provide skilled nursing care to residents under the supervision of a Registered Nurse (RN)
Administer medications and treatments as prescribed by physicians
Conduct vital signs, monitor patient conditions, and report changes to RNs and physicians
Assist with daily living activities, such as bathing, dressing, and grooming
Participate in care conferences and develop individualized care plans
Collaborate with interdisciplinary teams to ensure comprehensive care
Maintain accurate and detailed records of patient care and treatment

Requirements:

Current LVN license in the state of CA
Minimum 1 years of experience in a long-term care setting
Ability to lift, push, and pull 50 pounds with assistance
Excellent communication and interpersonal skills
Strong observation and problem-solving skills
Ability to work effectively in a fast-paced environment
