We provide whole person care by facilitating timely follow up and care coordination. What You'll Do Shift Scheduling: May start as early as 8AM or 10AM depending on the market. You will need to be available to work a mix of both early morning and late afternoon shifts for each 10-hour shift. Weekend Scheduling: You will be required to be available for assignment of at least four (4) weekend days during each 28-day scheduling cycle. Holiday Schedule: You are expected to work three (3) holidays annually.
