Eldercare Clinic Manager

Position Responsibilities: In collaboration with department leadership, reviews and revises department policies/procedures as necessary. Ensures the appropriate distribution and education of department policies/procedures. Ensures the management of data collection, aggregation and dissemination across the continuum, specific to clinical, financial and operational performance for designated area of responsibility. This includes, but may not be limited to, concurrent and retrospective chart audits as necessary. Maintains and monitors daily reports to ensure appropriate staffing levels based on unit of service standards Oversee the daily coordination and the implementation of the Eldercare PACE Model of Care. Direct, supervise, hire, fire and evaluate the performance of direct reports, including clinical oversight, education, training, reporting requirements and competency evaluations of the Center C.N.A.'s. In conjunction with the Quality Improvement Manager, maintains the stability and reputation of Eldercare by ensuring that all activities and operations are performed in compliance with local, state, and federal contracts, regulations, laws, accreditation, protocol, licensing and certification requirements governing operations. Assist in program development and implementation of Eldercare's strategic plan, including short and long-term goals and budgets. Participants or assigns a designee to all interdisciplinary team meetings, communicating participant changes, collaborating on care planning decisions and coordination of 24 hour care delivery. Serves as alternate facilitator. Oversee and direct the coordination of home health, rehabilitative, long-term care and medical/specialty services according to participant care plans. Manages flow of the clinic, oversees appointment bookings, triaging and gate keeping in conjunction with primary care and staff. Ensures timely and appropriate care of participants. Assists Director with evaluation of Eldercare's performance against national results and established service standards, clinical outcomes, utilization and other financial criteria. Establish and maintain positive working relationships with all members of the Eldercare team, the hospital and all Network Providers. Oversees and ensures signing off orders, ordering medications from pharmacy and disseminating orders and instructions to appropriate personnel. Ensures implementation of physician orders. Communicate with weekend and after hour staff, following up appropriately. Participates in the on-call rotation. Promotes effective communication and collaboration between the clinic and members of the enrollment, activity and business office. Participates in Leadership meetings to review risk scoring, pharmacy benefit managers Part D reviews, utilization, QI directives, Key Performance Indicator reviews and other key metrics of Eldercare at least monthly; addresses concerns with the Director from staff. Assists in improving these reviews and updating this process as needed. Conducts an in person comprehensive initial, regularly scheduled assessments and other nursing assessments of Eldercare enrollees, according to program protocols either directly or via delegation and oversight Perform or supervise general skilled nursing care to participants. Perform or supervise the administration of prescribed medications and treatments in accordance with nursing standards. Assist in treatment, examination and testing of participants. Assist with all aspects of hospital, SNF or ALF admissions. Insures coordination of participant care with outside contracted service providers – nursing homes, assisted living, lab, oxygen, etc. Qualifications: Current RN Licensure in State of Florida. BLS or CPR and ACLS required. Graduated from accredited school of registered nursing., BSN preferred Minimum of three (3) years of health care experience preferable in a geriatric physician office required. Experience in nurse management in managed care, hospital, nursing home or urgent care is acceptable for this position. A minimum of one year's experience working with the frail or elderly population is required. Either training or related experience in direction of a health care program. Position Responsibilities: This position will work in an Outpatient Oncology setting in Aventura, FL. Directs nursing service activities for the assigned unit. Has a 24 hour a day, seven days a week responsibility for the nursing operations within the respective unit. Ensures the highest level of clinical quality, customer service and cost effectiveness. Participates in and/or leads cross functional teams and is willing to make rapid and sound changes for the Medical Center to be a National and International leader in clinical outcomes. Plans, organizes and directs activities of the assigned unit through the management of the team leaders and charge nurses. Maintains budgetary/fiscal practices with provisions. Monitors budget expenses, identifies variances and reports as appropriate. Implements alternate solutions for the nursing unit to comply with budget. Ensures all processes and procedures provide for consistent, timely, high quality and cost effective clinical results. Responsible for maintaining and improving clinical care within the assigned unit towards the goal of being rated among the best in the United States. This includes but is not limited to: a. Maintaining all appropriate licenses and accreditations b. Timely reporting of incidents, with the appropriate review c. Correction of issues leading up to incidents in a timely and appropriate manner d. Monitoring of compliance through safety/other regulations established by the respective regulatory/accreditation agencies Provides leadership in complex and/or stressful situations, counsels staff based on factual and objective information. Ensures timely and appropriate actions for resolution. Actively participates in Medical Center re-organization initiatives, as required by top management. Proactively makes necessary changes to complete any departmental changes with minimal impact to patients, physicians and employees. Identifies staffing patterns to cover the unit's needs around the clock. Sets and monitors staff schedules in relation to patient acuity, census, technology and resources management to meet budgetary needs. Participates in cross-organizational activities to improve the performance of the unit and share the learning with the team. Supports the database information system. Maintains current knowledge of the system - understands data entry, maintenance and reporting. Establishes and maintains Medical Center safety and compliance policies. Qualifications: Licensure as a Registered Nurse in the State of Florida is required. BLS required. ACLS preferred. Graduate from an accredited school of nursing, BSN required. MSN preferred. Three years of Nursing leadership experience preferred. Position Responsibilities · Prepares the OR suite appropriately to receive a new admission. · Evaluates, assesses and documents patient’s surgical status. · Assists anesthesia in maintaining intake and output, checks availability of blood products and checks blood products with anesthesia before administering. Document blood administration and assures accuracy. · Demonstrates ability to handle difficult situations, and appropriate usage of safety measures. · Monitors, evaluates and intervenes appropriately with patients receiving conscious sedation. · Competent in application of safety positioning devices, providing ongoing observation, intervention and documentation as per hospital policy. · Maintains thorough knowledge of operation, care and handling of O.R equipment. Demonstrates ability to perform new procedures, skills and routines in specialty areas. · Performs initial vital signs and admission history to include introduction to patient and family. Assessment of chief compliant and notification of physician within 15 minutes of arrival to procedure area. · Obtains and documents an accurate systems assessment at the beginning of each procedure. · Responds to diagnostic data and initiates appropriate action or intervention. · Assesses patient education needs on admission, and psychosocial needs specific to patients age, growth, and development. · Initiates plan of care within eight hours of admission. Establishes priorities for patient care based on patient needs as evidence by completion of assigned duties within shift. · Performs Pre-Operative and Post-Operative care and instruction for each assigned surgical patient during shift. · Performs dressing changes using aseptic technique. · Collects and labels specimens per policy when ordered by physician. · Performs bedside glucose monitoring according to policy, performs ostomy care to include irrigation, appliance usage and reinforcement of teaching. · Inserts intermittent or indwelling urinary catheter using aseptic technique. · Administers oxygen therapy according to physician order. · Transfer patients in/out of the GI station in an efficient manner, by obtaining a complete and correct set of physician orders. Completes documentation of transfer summary. · All other duties as assigned.
