As a member of the clinical team, the Nurse Practitioner will bring an advanced knowledge and diverse skill-set to assess the healthcare of chronically ill geriatric members, in collaboration with physicians and interdisciplinary teams. Essential to this role is the desire and ability to aid and empower members in maximizing their quality of life, ensuring they can live as independently as possible in their homes and their communities. Responsibilities This hands-on role includes a host of responsibilities for these diverse members, including: Performing health histories and in-depth medical exams for new members, to determine preliminary Plans of Care Initiating, evaluating and reviewing medical orders, and identifying opportunities to improve care, when possible Carrying out various procedures and documenting all care and recommendations Ordering, interpreting and following up on lab tests and results Requesting consultations from medical and non-medical in-house and external providers, including physicians, therapists, dietitians, social workers and other specialists Educating staff, members, caregivers and families on essential care functions to ensure members have maximum independence at home and in centers Completing ongoing assessments to determine Plan of Care changes or improvements based on member feedback and medical evaluations Qualifications All applicants must be certified registered nurse practitioners, with a valid Pennsylvania nursing license. Additionally, candidates must be willing to participate in continuing education/workshops to maintain his/her valid license. A minimum of one year’s experience working with an elderly community is required, as is a strong passion to help this unique population each day. EOE

Position Overview LVPG OB/GYN is an enterprise of multiple practices across several counties in southeastern Pennsylvania that serves women’s preventive, gynecological, and obstetric needs. Inpatient care is conducted at 2 Family Birth and Newborn Centers, Cedar Crest (Allentown) and Muhlenberg (Bethlehem) hospital sites, and 2 primary surgical locations at Muhlenberg and 17th Street (Allentown) campuses. The OB/GYN team of physicians, APC, midwives supported by operations and clinical leadership work together for the benefit of our patients and colleagues. There are additional opportunities for professional engagement in clinical committees, teaching and administrative roles. In an obstetrics and gynecology career with LVHN, you will experience: Possible work hours of Monday- Thursday of 8AM-5PM, 7:20AM-4:20PM, or 7AM-4PM. Fridays 8:20AM-4PM Evening sessions of seeing patients 10-7 NO CALL 90% Outpatient and 10% Rounding/Consults Over 30 service sites for obstetrics and gynecology in 11 counties serving women’s preventive, gynecologic and obstetrics needs. Five Family Birth and Newborn Centers throughout the region provide maternity care. Additional locations provide gynecologic surgical care for women using traditional and advanced minimally invasive techniques. Multiple locations give physicians and APCs the option to choose the setting in which they practice.
