The Administration for Children’s Services (ACS) protects and promotes the safety and well-being of children and families through child welfare and juvenile justice services and community supports. ACS manages community-based supports and foster care services, and provides subsidized child care vouchers. ACS child protection staff respond to allegations of child maltreatment. In juvenile justice, ACS oversees detention, placement and programs for youth in the community. The Administration for Children’s Services (ACS) Office of Child and Family Health (OCFH) delivers direct medical services to children entering foster care and ensures that the physical and mental health care provided is comprehensive and appropriate. OCFH offers individualized case consultations and training to ACS and foster care agencies on a variety of health-related topics. The office also conducts research and provides clinical expertise in developing health and mental health policies that support best case practice agency-wide including Youth and Family Justice, and Child and Family Well-Being. Within the OCFH, the Pre-Placement Medical Services Unit is responsible for the day-to-day medical functions at the Children’s Center pre-placement program, and ACS’s borough office medical units (Bronx, Queens, and Brooklyn). Providing 24/7 coverage, the Unit completes physical assessments for all children entering foster care and direct primary care for a comprehensive range of symptoms and illnesses. Under the direction of the Pre-Placement Medical Director, Pre-Placement Staff Nurses will be responsible for but not limited to: - Arrange follow up appointments, referrals where appropriate - Verify that all children have necessary medications and equipment at the time of placement - Assess physical and mental health care needs of ACS served foster care children as they enter care and on an on-going basis by providing age-appropriate health services. - Provide comprehensive physical assessments and correctly identify the need for emergency treatment, next day follow-up care or ongoing medical needs. - Directly administer medications - Conduct consultations in safe medication administration and management to child- care staff and foster parents - Consult, refer, and collaborate with other disciplines involved in the care of children - Provide adequate and appropriate transfer of information when children are transferred to a placement or referred to another health care provider - As a member of a health care team and the broader professional team, develop/implement and monitor a comprehensive patient care program - Participate in departmental and inter-disciplinary conferences and meetings pertaining to policies and procedures affecting nursing practice - Implement treatment protocols, procedures, and other orders and instructions from the Pre-Placement Medical Director and other clinical authorities - Conduct patient education, training to new foster parents of medically fragile children, and collaborate with foster care agencies to assist with services for foster families who care for children with special needs - Train ACS and contracted agency staff on protocols, policies and medical conditions ADDITIONAL INFORMATION: Section 424-A of the New York Social Services Law requires an authorized agency to inquire whether a candidate for employment with child-caring responsibilities has been the subject of a child abuse and maltreatment report. POSITION OVERVIEW: The RN Case Manager is a direct link between medical services and the ICF/IRA residents and staff as well as with clinical professionals outside the program. The RN Case Manager is responsible for the complete nursing care and performance of all routine medical procedures within the scope of the nursing practice. The RN Case Manager is responsible for supervising LPN’s, AMAP and Direct Support Professionals (milieu counselors) in the performance of nursing tasks and activities. The RN must possess a working knowledge of OPWDD rules and regulations and JBFCS policies and procedures. The RN must be capable of supervising, understanding and working effectively with diverse populations. KEY ESSENTIAL FUNCTIONS: Provide direct nursing care as prescribed by the physician/Nurse Practitioner, in compliance with recognized nursing standards and State and Federal regulations. Uses nursing process to make continuing assessments of the individuals with developmental disabilities total health needs. Maintain direct interaction with consumers on a regular basis, but in no case, at a frequency of less than once per week. Perform nursing assessments, by direct physical examination, of each consumer upon entry into the program quarterly (at a minimum) thereafter. Develop and implement an individualized plan for nursing service. The nursing care plan must be updated at least annually or whenever there is significant change in the consumer’s condition. Coordinate all acute and ongoing health services, as indicated by need and regulatory mandate. Monitor health and behavioral status, labs and efficacy of prescribed treatments. Ensure timely communication of findings to appropriate clinical personnel. Ensure that all medical record documentation is accurate, timely and outcome based and reflected in the nursing notes. Provide initial and ongoing, but at a minimum frequency of annually, training to staff in all nursing tasks, functions, protective and preventive health measures that include but are not limited to: Appropriate health and hygiene methodologies Control of communicable diseases and infections Detection of signs and symptoms of illness, first aid for accidents or illness and basic skills required to meet the health needs of the consumers Client specific medical/health related issues Seizure management Ensure that training in the above noted areas is given to all new hires and per diems prior to their assuming client specific work assignments. Provide training and ongoing support to direct care staff regarding medication administration, tube feeding and diabetic care utilizing the standard curriculum approved by OPWDD. Conduct annual clinical performance evaluations for unlicensed direct support staff for procedures that include but are not limited to: medication administration, tube feeding and insulin administration. Evaluations must become part of the employee’s annual performance evaluation. Maintain documentation of all training curriculums, attendance and post training testing. Must have experience and ability in working with electronic health record management, electronic care coordination and electronic health record interface with external service providers. Responsibilities The Registered Nurse is responsible for the delivery of safe patient care utilizing the nursing process of assessment, planning, implementation and evaluation. Provides direct patient care within the scope of practice. What You'll Be Doing The Director of Nursing is responsible for the organization and the administration of the nursing services of the Hospital Rehabilitation Center. Administrative authority, responsibility, and accountability for functions and activities of the nursing staff. The Director of Nursing shall ensure that the total needs of the patients are met and shall keep the Administrator and Medical Director of the Rehabilitation Center informed of the activities of the nursing service and of any problems relating to the care of the patients. What We're Looking For 2-4 years supervisory experience in Long Term/ Rehab setting required Valid NJ RN License, BLS (Basic Life Support) Ability to lead, mentor, coach, and educate others as needed to accomplish projects. Polished communication and presentation skills both oral and written.
