As a per diem registered nurse at Atria, you will be providing personalized, exceptional health care to our members, both pediatric and adult. About The Role: Partner with Atria physicians and nurse practitioners to provide preventive, primary and urgent care to members in their home in the Westchester and Connecticut area Provide highly personalized nursing care activities for both adult and pediatric patients developing strong relationships with members Perform minor procedures, such as vaccinations, phlebotomy, insertion of peripheral IV for medication administration as prescribed/ordered under a provider’s direction. Perform point-of-care testing in the home such as rapid covid, flu, and strep testing Perform sample collections and processing utilizing the medical testing machines located at the Westchester/Greenwich office. Complete forms and paperwork for members as needed Flexibility to perform administrative and clerical tasks related to clinical practice such as inventorying medication, ordering supplies, or scheduling visits. Communicating with primary care and specialty clinical staff in a team based model of care. Provide education to members about atria member offerings such as medication kits and tools for telemedicine as well as about care for acute and chronic conditions i.e. anticipatory guidance on wound care. Triage member calls for possible sick visits. About the Role: We are seeking a full-time nurse practitioner or physician assistant to join our multidisciplinary clinical team in Palm Beach. In this role, you will provide proactive, longitudinal skin care—including full-body skin exams, early detection of skin cancer, and interpretation of Vectra 3D total body imaging—with the support of a world-class team and advanced diagnostics. You will also collaborate with internal medicine, pediatric and specialty colleagues in a modern, tech-enabled, high-touch environment to provide medical dermatology and case management for chronic skin conditions. Responsibilities Conduct early detection and prevention full body skin exams for adults by leveraging Vectra 3D imaging to identify changes, guide biopsies, and support timely intervention. Act as primary liaison with off-site dermatologists caring for our members to provide coordinated care and best-in-class medical care for skin lesions requiring biopsy, excision, or close follow up. Diagnose and manage common dermatologic conditions including warts, acne, eczema and other conditions. Participate in dermatology research initiatives with Vectra 3D technology, advancing data-driven, cutting-edge skin health and cancer prevention studies Coordinate closely with primary care clinicians and across disciplines to deliver integrated, personalized care Deliver care in multiple formats: in-clinic, via telemedicine, and occasional home visits Participate in ongoing innovation, quality improvement, and education Nurse Practitioner’s licenses in the state of FL Minimum of 2 years of clinical experience in medical dermatology required Proficiency in punch biopsy and minor procedures of medical dermatology preferred Licensed in FL (required) Dermoscopy experience (preferred) or willingness to learn dermoscopy Technology proficient and able to trouble shoot and work with the systems required for care Strong interpersonal skills and commitment to team-based care Work onsite 5 days per week Comfort working in a modern, tech-enabled practice Who You Are You are mission-driven and collaborative. About the Role: We are seeking Advanced Care Providers (Nurse Practitioners and Physician Assistants) to join our expanding Internal Medicine teams and collaborate closely with our Chief Medical Officers. Ideal candidates will demonstrate a commitment to providing high-quality patient care and bring an innovative, forward-thinking approach to their work. Candidates should possess strong internal medicine knowledge and have experience in urgent care, primary care, or family medicine, with a comfort level in treating the adult population. Essential Job Duties: Patient Assessment and Diagnosis: Nurse Practitioners and Physician Assistants (NPs/PAs) partner with CMOs to perform comprehensive health assessments, including medical history, physical examinations, and diagnostic tests, to identify patients' health status and needs. Diagnose acute and chronic health conditions, formulate differential diagnoses, and develop appropriate treatment plans. Treatment and Management: NPs/PAs collaborate with CMO to provide primary and specialty care services, including prescribing medications, ordering, and interpreting diagnostic tests, and implementing therapeutic interventions. Manage chronic conditions, monitor patients' progress, and adjust treatment plans as necessary to optimize health outcomes. Health Promotion and Disease Prevention: NPs/PAs educate patients and families about health promotion strategies, disease prevention measures, and lifestyle modifications to improve overall health and well-being. They emphasize preventive care, such as immunizations, screenings, and health maintenance activities. Collaborative Care: NPs/PAs collaborate with interdisciplinary healthcare teams, including physicians, nurses, and other healthcare professionals, to coordinate patient care effectively. They consult with specialists, refer patients to appropriate healthcare providers, and facilitate continuity of care across healthcare settings. Clinical Care: NPs/PAs will be required to perform clinical skills that include but are not limited to; phlebotomy, IV insertion, and EKG. May need to see patients independently for urgent visits on weekends. Documentation and Recordkeeping: NPs/PAs maintain accurate and up-to-date medical records, including patient histories, assessments, diagnoses, treatment plans, and progress notes. They will assist in completing any forms for patient care such as Radiology questionnaires. They ensure compliance with legal and regulatory requirements, as well as organizational policies and procedures. Ethical and Legal Compliance: NPs/PAs adhere to ethical principles and professional standards of practice, maintaining confidentiality, integrity, and respect for patients' rights and dignity. They comply with legal and regulatory requirements governing nursing practice, licensure, and scope of practice.
