Why There is So Much Documentation Required to Work in Long-Term Care

If you have been through the IntelyCare onboarding process or are just starting your journey with us, you are probably aware that long-term care requires a lot of credentialing. Long-term care facilities support some of the most vulnerable populations, including the elderly, who are particularly susceptible to communicable and non-communicable diseases, injuries, mental illness, and other serious conditions.

1. Identification and Verification

These documents are used to verify your identity, work authorization, and if required by the state regulatory body, perform a background check.

  • Government-issued Photo Identification, such as a driver’s license, state-issued ID, or passport.
  • Social Security Number, which permits you to work in the U.S.
  • I-9 Form verifying your eligibility to work in the U.S.
  • Your Signature, for verification.

2. Professional Qualifications

These documents are important because they assure healthcare facilities that you have maintained the necessary training and skills to care for patients and yourself and achieve positive health outcomes.

  • An Active Professional License in the state you intend to practice.
  • Active CPR Certification from the American Heart Association (AHA) or the American Red Cross.
  • Dementia Training for certain long-term care facilities in Pennsylvania and Massachusetts.
  • HIV Prevention Training from a board-approved facility (if you choose to practice in Florida).
  • And a Skills Checklist to gauge your abilities within your healthcare specialty.

3. Health Records

To reduce the risk of injury and the transmission of communicable diseases, we require certain health records. It is crucial that we take proactive, cautionary measures to protect not only you but the vulnerable people you are caring for, such as the elderly and chronically ill.

  • Annual Physical.
  • Annual Tuberculosis Test.
  • And a Flu Shot and/or Drug Screening may be required to work in some facilities.

4. Employment Documentation

Last but not least, we require standard employment documentation.

  • W4 Information, which allows us to determine how much money needs to be withheld for federal income tax.
  • Direct Deposit Information to reduce risk and ensure timely payments.
  • And two Professional References for additional community insight and understanding of who you are, your clinical competencies, and how you have grown professionally.
