Position Summary The Inpatient Skin and Wound Program Nurse provides direct patient care and serves as a clinical resource/ consultant for patients with selected disorders of the gastrointestinal, genitourinary and integumentary systems. The Inpatient Skin and Wound Program Nurse should be prepared to provide the full scope of enterostomal therapy nursing practice, including education, counseling, research and administration. Responsibilities Utilizes the nursing process to provide direct patient care in the inpatient setting for wound and Utilizes the nursing process to provide direct patient care in the inpatient and outpatient settings for the population. Serves as a consultant in the areas of expertise. Educates patients, families and members of the interdisciplinary team in areas of expertise. Supports Inpatient Skin and Wound Program maintenance and advancement. Maintains and advances knowledge and skills in areas of expertise. Minimum Requirements Bachelor's Degree in Nursing is required, MSN is preferred. 3 to 5 years of clinical nursing experience. MSN may be substituted for 1 year of experience. Current RN licensure is required. Current Wound and Ostomy certification by the Wound, Ostomy, Continence Nursing Certification Board (WOCNCB) is preferred. ESSENTIAL JOB FUNCTIONS: • Utilizes the nursing process by demonstrating appropriate assessment, planning, implementation and evaluation of residents wound and skin integrity • Collecting and documentation of residents’ skin status including medical history. • Wound care prevention and prevention through plan of care • Sets priorities for nursing action for wound care and prevention • Communicates with physicians regarding resident wound needs • Initiates nutritional interventions to ensure that residents with wounds are provided nutrition to promote wound healing. • Performs direct resident care utilizing established procedure, policies, and standards • Administer treatments and procedures in accordance with the physician’s orders • Maintains the comfort, privacy, and dignity of each residents while performing treatments • Communicates and interacts effectively, courteously, and tactfully with residents, visitors, family members, peers, and supervisors • Conducts rounds frequently to ensure that residents with incontinence are receiving pericare and preventative ointments timely. Wound Care Nurse Practitioner / Physician Assistant Job Title: Wound Care NP/PA Department: Wound Care Reports To: Supervising Physician / Medical Director Job Purpose The Wound Care NP/PA provides direct patient care under the supervision of a wound care physician. This role is responsible for assessing, diagnosing, and managing acute and chronic wounds, performing wound-related procedures, and educating patients and families. The NP/PA ensures compliance with state scope of practice laws, organizational policies, and regulatory standards. Essential Duties & Responsibilities Conduct patient assessments, histories, and physical examinations focused on wound care. Perform wound-related procedures, including debridement, dressing selection/application, and use of advanced wound care therapies. Order and interpret diagnostic studies related to wound care. Develop and implement care plans in collaboration with supervising physician. Provide patient and caregiver education regarding wound care and prevention. Accurately document clinical encounters in the EMR in accordance with coding and compliance standards. Participate in interdisciplinary care planning and team conferences. Maintain professional development, certification, and continuing education requirements. Adhere to organizational policies, infection control standards, and HIPAA compliance.
