We are currently seeking a Wound Care Nurse Practitioner to provide wound services to patients living in long-term care facilities.

PRIMARY AND WOUND NP ROLE AND RESPONSIBILITIES:

  • Provide wound care and primary services for long-term care patients residing in skilled nursing facilities.

  • Prescribe appropriate medications, including controlled substances in accordance with OH Law.

  • Order, review, and/or follow-up with treatments and diagnostic testing.

  • Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.

  • and/or the care plan.

  • Collaborate with the primary care physician.

PRIMARY AND WOUND NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

  • Current, unrestricted NP license in the state of Ohio.

  • Board Certification.

  • DEA licensure (Optional)

  • Clinical background in adult, family, or geriatric specialties.

  • Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds

  • Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.

  • Must be comfortable staging pressure injuries according to the NPIAP

  • Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.

  • Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices

  • Strong preference to having wound certification or be open to getting certified 

  • Experience working in a long-term care setting.

  • Ability to work with an EMR.

  • Ability to work within an interdisciplinary care team to coordinate patient care.

  • Basic coding knowledge preferred, not required.
