The Psychiatric Nurse Practitioner Residency includes: Enhanced supervision and support, the heart of our NPP Residency Program. Residents spend the brunt of their time in precepted clinical experiences, where they receive dedicated supervision from seasoned NPPs and psychiatrists who provide individualized supervision throughout the day, including evenings. Residents also work in specialty rotations, where they gain experience providing care to a diversity of clients, with regular supervision and support. In all clinical experiences, residents gradually build their caseloads to avoid the “sink or swim” situation that newly graduated Psychiatric Nurse Practitioner Residents typically encounter. Our Psychiatric Nurse Practitioner Residency Program encourages leadership and the ability to tackle larger issues related to mental healthcare. Residents take part in weekly didactic sessions to review and discuss stimulating readings, guidelines, case studies, and other learning materials to supplement their clinical development as Psychiatric Nurse Practitioners. Job Summary The Psychiatric Nurse Practitioner provides behavioral health care services to patients under the direction and supervision of a physician. This position provides coordination and collaboration of care planning for behavioral health patients with co-morbid conditions, as assigned. Essential Functions Provides acute and chronic patient care including taking histories, evaluations, monitoring therapies, giving injections, and prescribing medications. Performs a thorough and timely assessment of patient’s condition including physical, psychological, social and rehabilitative aspects. Follow Up visits for the patients after medical appointments or transfers. Provides education to patient and family on managing the condition, and coordinates care with internal and external disease management or case management entities as necessary. Develops and implements patient care plans, instructs, and counsels patient, and records progress in electronic medical records systems. Documentation on the patient’s medical record is accurate, complete and concise Attend regular meetings with psychiatry to foster collaboration in practice management. Provides presentation and educational offerings in department as assigned. Provides training to clinical staff on new policies, protocols, and procedure as appropriate. Identifies problems/nursing needs including discharge needs Intervenes appropriately to address identified problems and needs. Plans care according to standards of care. Manages time efficiently/sets priorities appropriately Evaluates effectiveness of care and revises plan if necessary Provides education consistent with the patient/family learning abilities and preferences; evaluates outcomes Effectively communicates with physician’s, co-workers, patients, families Reports pertinent observations regarding patients to physicians and other health care team members Maintains National Patient Safety Goals Verifies, coordinates and carries out physicians orders accurately Assists with coordinating interdepartmental transfers smoothly and efficiently Assists with interdepartmental activities (i.e., lab, radiology, etc.) to ensure timely efficient care Communicates and coordinates with Case Manager to ensure timely discharge, patient satisfaction and quality care along the continuum of care Practices Universal Standard Precautions Demonstrates knowledge of practices department safety procedures, including safe handling of equipment and hazardous substances Performs other job-related duties as required. POSITION OVERVIEW: The psychiatric nurse practitioner (NPP) is responsible for providing psychiatric services and care to the agency's children and adult population in community clinic practices, including clinical diagnosis, prognosis, medication management, and discharge planning. The NPP functions as an integral part of the interdisciplinary team environment approach of the Agency. The NPP may be asked to cover for other NPPs and/or assist with administration of long acting medication when nurse unavailable. KEY ESSENTIAL FUNCTIONS: Provide psychiatric assessment, clarification of clinical diagnosis, recommend treatment options and provide client education on mental health issues and strategies for recovery and review patient's progress and recommend discharge and aftercare planning. The former is needed to prescribe to clients with Medicaid, the latter is needed to adhere to NYS I-STOP law. Must be able to work as a team member with complex cases. Working knowledge of Microsoft Office, ability to learn electronic health records and other software as required. EXPERIENCE REQUIRED/LANGUAGE PREFERENCE: Russian required.
