JOB SUMMARY Under the administrative supervision of the Executive Director or designee, provides general medical care and treatment to recipients in collaboration with the physician. Responsible for the primary health care of assigned consumers. 2. Performs history and physical examinations and preventive health measures within prescribed guidelines in collaboration with a physician. 3. Orders, interprets, and evaluates diagnostic tests to identify and assess recipients' clinical problems and health care needs. Orders laboratory tests, x-rays, and other diagnostic procedures. 4. Observes recipient progress, records physical finding and formulates plan and prognosis, based on recipients' condition. Discusses case with physician and other health professionals to prepare comprehensive recipient care plan. 5. Submits health care plan and goals of individual recipients for periodic review and evaluation with the physician. May recommend changes to the plan of care. Prescribe or recommend medication or other forms of treatment such as physical therapy, diet, inhalation therapy, or related therapeutic procedures. Prescribes or recommends continuation or discontinuation of medications and treatment. 6. May refer recipients to physician for consultation or specialized health resources for treatment. 7. May assist the psychologist in performing mental status assessment and behavioral evaluations. Performs crisis intervention using verbal and nonverbal techniques. 8. Immediately reports serious incidents, serious incident allegations, incidents, or sensitive situations to supervisors. 9. Assists in the development of nursing policies and procedures and in the orientation of new nurses. 10. Compliance with attendance rules and the ability to work on a regular basis. 11. Attends regularly scheduled clinical meetings and supervision, attends professional development activities and training. 12. Performs other job-related duties as assigned. ADDITIONAL TASKS: 1. May conduct workshops for recipients and staff on health-related issues to include health maintenance, medications, and medication administration. 2. May precept nursing students and nurse practitioner students. 3. May be assigned as a chair or part of an agency committee(s). 4. May serve as the agency Bloodborne Pathogen Coordinator and teach the Bloodborne Pathogen Standard workshop and makes recommendation concerning the agency' infection control posture. May conduct and interpret Mantoux Tests (TB). QUALIFICATIONS AND EXPERIENCE Master’s degree in Nursing from an accredited nurse practitioner program, and currently licensed in New York State as a family practice nurse practitioner. Current DEA certification. ANCC certification is an asset. Two years’ experience providing nursing services preferred. Must be eligible for full and unconditional participation in the Medicaid and Medicare programs. May be required to possess a valid NYS driving license and a clear driving record. Continued employment is based on the ability to maintain professional licensure, registration, accreditation, and participation in the Medicaid and Medicare programs.

Under the supervision of the Registered Nurse and Program Director, and as a member of an inter-disciplinary treatment team, assumes case management responsibilities for assigned caseload of individuals who have mental illness and substance abuse disorder. Renders nursing care as assigned by registered nurse in accordance with recognized nursing techniques, procedures and established standards based on the scope of practical nursing. THIS IS A FIELD OPPORTUNITY ESSENTIAL TASKS: Assumes case management responsibility of assigned caseload of individuals. Assertive engagement and provision of mental health services in the community where consumer lives- up to 80% of weekly work schedule may involve field work. Continuous responsibility (24/7) for individuals engaged in the CB ACT or BUCSC ACT program. Assists individuals in assigned caseload to see the relationship between their substance abuse and their problems in living, the process of motivating the client to accept abstinence-oriented treatment. Provide assistance to consumers in structuring and performing basic daily activities. Assists consumer in the development and support of social activities and relationships. Provides support to the program participants’ families and significant others. Provides intensive housing supports. Maintains accurate and timely case records, including but not limited to: weekly progress notes, quarterly service plan reviews, assessments and medical records. Provides and/or arranges for emergency medical care. Assesses consumers’ health condition on a daily basis. Effectively communicates consumers’ changing needs and plan of care among team members.
