GENERAL SUMMARY: The Registered Nurse is responsible for the planning, delivery and management of direct patient care using the nursing process and adhering to the standards of nursing practice within a specific unit or clinical area. PRINCIPAL DUTIES AND ESSENTIAL FUNCTIONS: Nursing Process: Utilizes the nursing process for the delivery and management of patient’s care. Maintains assignment of patients and caseload according to unit guidelines and established standards of practice. Uses positive customer service skills when communicating with patient and patient’s family. Utilizes the nursing process for the delivery and management of the patient’s care. Utilizes clinical reasoning and critical thinking. Involves patient, family/significant others and other disciplines in decision making related to the patient’s plan of care. Serves as a patient and family advocate. Implements interventions consistent with hospital standards of practice and prescribed medical plan of care and evidence based practice . Documents the patient assessment, plan of care, and response to intervention. Meets all unit documentation standards. Communicates and coordinates the patient’s plan of care to ensure continuity of care. Maintains competency in management of clinical emergencies. Provides complete communications in SBAR format (Situation, background, assessment, recommendations) to relieving staff members and demonstrates advanced planning for the patient. Navigates through the critical elements of the patient medical record software and HealthAlliance-Clinton Hospital Intranet. Clinical Practice: Demonstrates/applies the skill and judgment necessary to implement the medical/nursing plan of care, nursing interventions and procedures necessary for the care of the patient. Bases clinical practice on evidence, unit guidelines, and professional standards to improve the quality of care and to guide clinical judgments. Demonstrates essential knowledge of pharmacology/medications in administering drugs and monitoring the patient’s response. Maintains competency with technology and equipment. Utilizes equipment in a safe manner. Uses appropriate infection control and safety practices in all duties. Acts as a charge nurse as requested by the nursing management team. Collaboration: Delegates responsibilities to others according to Massachusetts general laws and practice guidelines with an understanding of their roles, knowledge and capabilities. Certified Nursing Assistant

Overview $20.50/hour The Certified Nursing Assistant is responsible to provide assistance to the support staff with the daily activities of individuals, provide ongoing support, guidance, role modeling and providing assistance in meeting clients medical and health need. Minimum Education Required High School Diploma/GED Shift First Shift Additional Shift Details Monday-Friday 9am-5pm Responsibilities Provide support in a manner conducive to their safety and comfort, under the direction of an RN and Program Manager. Perform all duties in accordance with established methods, techniques and recognized standard. Assist program support staff in completion of all daily/shift tasks, including but not limited to oversight of individuals’ medical and health needs: hospitalization, appointments, assessments, discharging, transferring and caring for the residents' personal belongings. Provide ongoing support, guidance and role modeling for individuals. Assist individuals with home maintenance and perform cleaning duties as assigned. Perform Activities of Daily Living (ADLSs) for residents as required. Coordinate scheduling for residents requiring occupational therapy, physical therapy and speech therapy. Take and record weights, blood pressures, temperatures, pulse and respiration and give skin care treatments, as permitted by state regulation. Report on resident status or family concerns to RN and Program Manager. Ensure all paperwork, reports, trainings are up to date and completed within the Divisions expected timelines. Ensure that residents and families receive the highest quality of service in a caring and compassionate atmosphere, which recognizes the individual's needs and rights. Attend and actively participate in all trainings as assigned and maintain certification in CPR, First Aid, crisis management, and MAP Qualifications Certified Nursing Assistant Certificate Required. Experience with the program population is preferred. Ability to write routine reports and correspondence. Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations. Ability to read and interpret documents such as policy and procedure manuals. Strong computer knowledge. High energy level, superior interpersonal skills and ability to function in a team oriented environment and as well as work independently Ability to execute a variety of decision-making models. Commitment to Advocates values and mission. Must hold a valid drivers’ license. Must have access to an operational and insured vehicle and be willing to use it to transport clients.
