Position Summary:

Primary Nurse ESP Revere PACE-replace Ngenda Mweni Per diem

Per diem RN position available in our PACE Center, which provides integrated medical, rehabilitation, case management, and adult day health services all in one location. Responsible for day-to-day nursing assessment and for the delivery and documentation of quality patient care.  Current MA RN License required with minimum of 1 year of experience in ambulatory, home health, hospital or sub-acute setting preferred. A minimum of one year experience working with a frail or elderly population. Prior experience in interdisciplinary team setting a plus. Schedule: per diem
