Mass General Brigham Home Care provides non-acute, medically necessary skilled care in the home to help “homebound” patients recover from illness or injury and manage exacerbations of chronic disease. Services include skilled nursing care (IV drug administration, injections, tube feeding, wound care, care plan education), physical therapy, occupational therapy, speech language pathology, medical social services, and as needed medical supplies. We are seeking a part-time, 24-hour LPN to join the “Weekends” team, supporting territory South of Boston. Area would cover lower Middlesex, Suffolk and Norfolk counties. Our LPNs do not manage patients, but they work at the top of their license as adjuncts to the RN Case Managers to provide 1on1, complex nursing care and cutting-edge treatments, as well as education and support to patients and their care givers. NURSE PRACTITIONER MS: Outpatient Colorectal Surgery/ Full Time/ 40 Hours/ Foxboro/ Jamaica Plain/ Boston ADDENDUM for Colorectal Surgery Working in collaboration with the Colorectal Section Chief, GGI Surgery Administrative Director, the Colorectal Surgery NP is a valued member of the colorectal practice team. NP responsibilities include clinical practice on an interdisciplinary care team for patients receiving ambulatory care through the Department of Surgery, Division of Colorectal Surgery. The NP’s primary role will be focused on the larger colorectal surgery patient population as an advanced practice nurse. The NP will have a supportive role in treating patients who have or are about to undergo the formation of a stoma. The role requires significant interdisciplinary teamwork in acute and community settings. Clinical: Works in collaboration with the attending physicians: - Clinic schedule will include sessions primarily located at, but not limited to: Surgical Clinic at 21 Patriot Place, Foxboro, MA, Clinic sited at BWFH / Faulkner Hospital at 1153 Centre St, Jamaica Plain, MA, and Boston Main campus as needed. - These sessions may be held alongside our colorectal surgeons but also may be independent in their absence. The practitioner has the credentials to function in an expanded nursing role and is responsible for the daily management of selected patients. The practitioner has responsibility for assessment and management of patients including diagnostic and therapeutic interventions, and continuous implementation and evaluation of an appropriate plan of care. The Nurse Practitioner works under the supervision of the attending physician and is responsible to the Nurse Manager, or Director for the area. NATURE AND SCOPE OF PRACTICE: 1. Performs and documents medical, physical, psychosocial and nutritional assessment on both new and established patients. Documentation requirements include electronic medical record documentation, dictations, medication reconciliation, and CWMW database entry for all patients. 2. Evaluates CWMW patients to establish candidacy for a broad range of weight management interventions including medical nutritional management, lifestyle interventions, obesity pharmacotherapy, endoluminal procedures, bariatric surgery, and investigational options under study in our institution. 3. Evaluates CWMW patients for undiagnosed obesity related co-morbidities that may impact their overall health and/or participation in weight management treatment options. 4. Educates patients regarding available weight management interventions that are currently being utilized in clinical practice. 5. In conjunction with supervisory staff, the NP will assist in gathering and interpreting data collected from patient visits and diagnostic studies in order to formulate and implement plans of care as well as to provide follow-up to assess patient outcomes. 6. Evaluates for and initiates specialist consults as deemed appropriate. 7. Triages patient inquiries related to their CWMW care. 8. In conjunction with the attending physician, the NP identifies and provides appropriate treatment for any complication related to weight management interventions. 9. Job Summary Summary Working under the direction of licensed personnel, assists patients with activities of daily living tasks such as bathing, dressing, and eating. Also assists with taking patients' temperature, blood pressure, pulse, and respiration. Does this position require Patient Care? Yes Essential Functions -Performs vital signs, weights/heights, intakes, and outputs with accuracy and within established time frame and/or as requested. -Completes all documentation in the medical record as required. -Interacts with patients and their families effectively. -Helps patients with a number of basic hygiene tasks. This may include bathing and dressing patients, helping them use the toilet if needed, and feeding them. -Assist with helping patients turn and reposition themselves in bed as needed and help them move between the bed and a wheelchair if they use one. -Listens to any health concerns patients may have and report those concerns to the nurses.
