Franciscan Health Indianapolis Campus 8111 S Emerson Ave Indianapolis, Indiana 46237 WHO WE ARE LOOKING FOR: Franciscan Health is seeking an Advanced Practice Provider to meet the growing demand for Bone Marrow Transplant services at Indiana Blood & Marrow Transplantation located at the Franciscan Cancer Center in Indianapolis, IN. APP will be joining an established, collegial team of three physicians and two APPs. Indiana Blood & Marrow Transplantation (IBMT) program is one of only two FACT accredited programs in the State of Indiana performing over 100 transplants a year. The program offers both Allogeneic & Autologous transplants as well as cellular therapy for hematological malignancies including leukemia, lymphoma, and multiple myeloma as well as autoimmune disorders. IBMT is a National Marrow Donor Program (NMDP) collection center for Bone Marrow & Peripheral Blood Stem Cells. There is a Stem Cell processing laboratory on site. The program has both inpatient and outpatient services. Immune Effector Cell Therapies and Clinical Research are also part of the program. WHAT WE NEED: Bone Marrow Transplant, Hematology and/or Oncology experience preferred, but not required. Ideal candidate would thrive in a fast-paced environment and be team oriented. The candidate will need to work collaboratively with physician to coordinate and deliver the highest quality of patient care. Candidate should have a desire to develop exemplary professional relationships with patients and the community in which they serve. How you’ll make an impact in this role Manage the recovery of our most vulnerable patients by implementing complex care plans and maintaining constant vigilance over physiological changes to ensure immediate, life-saving interventions. Job Description: Job Summary: The incumbent of this position is responsible for ensuring quality services are delivered to clients/patients through providing clinical oversight to clients/patients and team members in a program by planning, organizing, staffing, and directing client/patient care on a daily operational basis. Essential Job Functions: Under leadership's guidance, will establish and maintain effective working relationships with other agencies in the communities served. Participate in selection of off-site resources for needed services that are not provided by the program as well as keep other agencies informed about the services offered by the organization. Complete monthly/quarterly record reviews for the program, ensuring that regulatory and accreditation standards are being maintained as well as monitoring compliance with program and agency Quality Improvement Indicators. Follow up with any deficiencies during weekly staff meetings and assist team members with correction of deficiencies. Facilitate weekly clinical case staffing, team and diagnostic disposition meetings. Act upon reports and recommendations from program/agency committees, team members and client suggestions to ensure that provision of services is appropriate, adequate and of high quality. Ensures transition planning occurs for all clients/patients Inform Leadership of all relevant client/patient issues. Maintain required weekly contact hours as applicable: conduct intakes, (demonstrate ability to determine individual needs of the client through completion of initial treatment plan), individual, family and group therapy determined by program and individual site needs. Complete and review written reports to referral sources as needed in a timely manner. Promote integration of clinical, community support, and physical healthcare services working as a team leader. Provide oversight for service delivery, completing weekly checks on team member direct service hours to ensure that the services rendered are: appropriate and clinically justified. Client patients are receiving appropriate frequency and intensity of services Team Members are meeting expected contacts and/or direct service expectations Monitor progress notes for thoroughness and completeness. Maintain necessary file documentation for services provided. Develop evolving programmatic schedule to meet the needs of the client population while ensuring that client rights and responsibilities are respected. Coordinate services and collaborate with outside agencies to provide comprehensive services to the client/patient and client’s/patient’s family. Adhere to and monitor program budget by reviewing service delivery plans, managing services that pertain to expenditures of allocated funds and review units of service for each client. Monitor and follow up, if needed, with all crisis calls occurring outside of program hours. Complete team member evaluations in a timely manner assessing competencies and noting strengths. Provide clinical direction and supervision to assigned team members, and identify performance strengths and weaknesses, performs corrective coaching in a timely and effective manner and maintains weekly supervision records as well as individualized training plans for each direct report. Ensure that orientation, in-service training and continuing education for staff members are met and that staff members receive training necessary to improve working knowledge of designated job duties to enhance the quality of service provision. Ensure that all policies and procedures are being implemented to guide and support the provision of services provided. Recommend any policy and procedure changes to assigned Leadership. Experience and Education Qualifications: Substance Use Disorder Behavioral Health Programs Graduate from an accredited college or university with a Master’s Degree in social work, counseling, psychology, psychiatric nursing, or a closely related field, who has at least three (3) years professional experience in the treatment or Behavioral Health of substance use disorder treatment and 1 year of supervisory experience in substance use disorder treatment; OR Graduate from an accredited college or university with a Bachelor’s Degree in social work, counseling, psychology, psychiatric nursing, or a closely related field, who has at least four (4) years professional experience in the treatment or Behavioral Health of substance use disorders and two (2) years of supervisory experience in substance use disorder treatment: AND Qualified Addiction Professional certified by the Missouri Credentialing Board, Inc. Or applicable State Board with four (4) years professional experience in the field of substance use disorder treatment and three (3) years of supervisory experience in substance use disorder treatment. OR Counselor, psychologist, or physician licensed in Missouri who has at least one (1) year professional experience in the treatment or Behavioral Health of substance use disorder treatment; AND at least one (1) year of supervisory experience in substance use disorder treatment. A qualified mental health professional meeting one of the following criteria: Physician licensed under Missouri state law to practice medicine or osteopathy with training in mental health. The Care Manager provides hands on care and physical and emotional support as outlined in each resident’s Individualized Service Plan (ISP) while maintaining a safe and comfortable home like environment. RESPONSIBILITIES & QUALIFICATIONS Essential Duties As a part of the Sunrise team, supporting our Mission, Principles of Service and Core Values is a fundamental part of this job. The unique responsibilities for this role include but are not limited to the essential functions listed as follows: Resident Care Review, read, notate, and initial Daily Log to document and learn about pertinent information about residents. Participate in the development of the ISPs and monthly updates. Review designated assignments. Responsible for a designated group of residents during the shift, knows where residents are, and physically checks on them throughout the shift. Observe, report, and document symptoms and conditions of residents for changes in condition such as skin, behavior, alertness, weight, diet, and participation in activities. Attend daily Cross Over meetings by the lead care manager. Notify supervisor and resident care director if a resident has increased care needs. Inform supervisor of any resident changes in condition. Respond to security system and resident call bells promptly and immediately. Take appropriate action including resetting call bells. Greet guests, family members, residents, and team members. Answer, direct and/or respond to phone calls in a timely, courteous, and professional manner. Assist with continence management and dispose of all continence products properly to ensure sanitation of resident suite and community restrooms using standard care procedures. Communicate with families and is a resource as needed. Help residents maintain independence and physical safety and promote dignity of each resident adhering to the standards of Resident Rights and Sunrise Principles of Service. Participate/lead and assist residents with activities of daily living (ADLs) and Invite, Encourage, and Assist (IEA) residents to participate in activities as indicated on the ISP. Engage residents in life skills and other life enrichment activities throughout the day in reminiscence. Strive to understand and respond to each resident with empathy, always remaining mindful of the resident’s unique communication patterns or history and basic human needs. Maintain an atmosphere of warmth, personal interest, and positive emphasis as well as a calm environment. Ensure the established safety regulations are always followed. Practice routinely good Standard Care Precautions of cleanliness, hygiene, and health. Host and engage in activities with the residents daily. Risk Management and General Safety Partner with community team to ensure community is in compliance with national/provincial regulations pertaining to occupational health and safety requirements, promoting Risk Management programs and policies, and adhering to safety rules and regulations. Practice safety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS), and Lockout Tagout procedures. Report all accidents/incidents immediately. Reports all unsafe and hazardous conditions/equipment immediately. Ensure any cords, carts, equipment, and other hazards are always kept out of the way, not blocking exits, and in compliance with fire codes. Understand and practice the proper method of attending to and disposing of, and the possibility of exposure to, blood borne pathogens, bodily fluids, infectious waste, sharp sticks, and hazardous materials. Report occupational exposures to blood, body fluids, infectious materials, sharp sticks, and hazardous chemicals immediately. Ensure oxygen tanks are stored safely, exchanged when empty and monitored to make sure liters of oxygen are at prescribed levels. Housekeeping and Laundry Services Maintain and clean resident’s room as assigned including making beds, emptying trash, putting clothes away appropriately, spot sweeping, and mopping as needed. Wash resident’s laundry as noted in the ISP and as needed. Wash and fold dining room linens and napkins. Complete assigned housekeeping tasks. Maintain common areas in a clean and tidy manner at all times. Dining Service Serve meals in the dining room and work in the dining room as assigned. Promote and ensure a pleasant dining experience during all meals. Assist with dining room set up and clean up as assigned. Participate in pre-meal meetings. Follow residents’ diets as indicated on ISP and Confidential Diet Board/Chart. Observe, note, and document in daily log any resident changes in dining habits. Respond to the dining needs of the residents and guests while maintaining Sunrise hospitality and service standards. Provide room service delivery as needed. Practice safe food handling in compliance with universal care precautions at all times. Specific Responsibilities for the Reminiscence Neighborhood Care Manager IEA residents in life skills and other life enriching activities as indicated on ISP and demographic profile. IEA residents to attend the afternoon social. Incorporate the concept of ‘Creating Pleasant Days’ into the resident’s daily routine. Integrate the individual resident’s life skills into their daily routine. Blend a variety of multi-sensory experiences into the resident’s day. Participate in monthly letters home and letter writing with the residents and their families. Training and Contributing to Team Success Participate as a member of a team and commits to working toward team goals. Demonstrate in daily interactions with others, our Team Member Credo. Commit to serving our residents and guests through our Principles of Service. Contribute to the overall engagement programs and processes (customer and team member engagement) including participating in the team member engagement survey and engagement improvement planning workshops. Attend regular meetings; Town Hall, Department Team, Cross Over, and others as directed by the Supervisor/Department Coordinator. Attend regular training by Assisted Living Coordinator (ALC), Resident Care Coordinator (RCC) or Reminiscence Coordinator (RC) using mini modules. Comply with all infection control techniques, placement of bio-hazard containers, and removal techniques as listed in policies and procedures. Maintains compliance in assigned required training and all training required by state/province or other regulating authorities as applicable to this role to ensure that Sunrise standards are always met.
