Nurse Practitioner- Opioid Clinic- Full Time

Horizon Health is seeking a Nurse Practitioner to join the team at Valle Vista for our opioid clinic called New Vista in greenwood, IN. The OTP Nurse Practitioner (NP) will work under the supervision of the Opioid Treatment Program (OTP) Medical Director to provide comprehensive care to patients in an OTP. The NP will perform patient assessments, intake patients into the program, and carry out other standard functions of a Nurse Practitioner within the opioid treatment setting. The OTP Nurse Practitioner will conduct thorough patient assessments, including medical history, physical examinations, and diagnostic tests. The OTP Nurse Practitioner will also perform patient intake procedures, including initial evaluations and development of individualized treatment plans and administer medication-assisted treatment (MAT) for opioid use disorder, including prescribing and managing medications such as methadone, buprenorphine, and naltrexone.

 

Responsibilities:

  • Monitors patient progress and adjust treatment plans as necessary.

  • Educates patients and their families about opioid use disorder, treatment options, and recovery strategies.

  • Collaborates with the multidisciplinary team, including physicians, counselors, and social workers, to ensure coordinated and comprehensive care.
  • Maintains accurate/up-to-date patient records in compliance w/ federal/state regulations.

  • Participates in quality improvement initiatives and continuing education to stay current with best practices in opioid use disorder treatment.

  • Addresses barriers to treatment, such as stigma and financial constraints, by providing supportive, nonjudgmental care.
  • Provides consultation to after-hours medical or mental health emergencies.

  • Provides supervision of Dosing Nurses as necessary.

  • Monitors and evaluates patients in recovery & provide interventions that will eliminate or reduce Substance Abuse.
  • Consistently and objectively intervenes with staff to correct employee performance that does not meet expectations using coaching or disciplinary counseling techniques.

  • Conducts and maintains minutes of monthly departmental meetings.

  • Develops and maintains staff schedule that is consistent with facility and departmental needs.

  • Develops and maintains clinical staff who are competent to provide departmental services.
  • Coordinates discipline-specific performance improvement activities as indicated.

  • Participates in hospital-wide performance improvement activities as indicated.

  • Completes and/or monitors time and attendance for clinical staff in compliance with Foundations Health policy and procedure.

Qualifications

  • Master's Degree and current NP licensure in Indiana required
  • Experience and/or speciality training in addiction medicine or addiction psychiatry required
  • A working knowledge of accreditation and licensing standards associated with the CARF, DEA, DBHDS, DMAS/ARTS, SAMHSA and Board of Pharmacy preferred.
  • A valid driver's license and proof of insurance required

Job Summary : PA or NP seeking supplemental hours are encouraged to apply for this PRN opportunity. We are open Monday through Friday 8a-8p and Saturday/Sunday 8:30am - 4:30pm. PRN Hours could include full shifts and/or 6-8 hour partial shifts. Prior urgent care, emergency care, or primary care experience preferred. The role of the Physician Assistant is to functionally support and extend the role of the Physician by providing patient-focused evaluation and communication. Provides care for specific patients under the supervision of and in collaboration with the Physician. At all times, the Physician Assistant will function in accordance with the applicable articles of Indiana Code regulating the practice of the Physician Assistant. As changes to Indiana Code are advanced, the Physician Assistant will comply with those requirements as well. Under current Medical Staff Rules and Regulations, all medical record entries and/or orders written by the Physician Assistant require later Physician authentication. Additionally, all patients must be seen and evaluated by the physician of record at least once every 24 hours (inpatient only). A Physician will directly supervise procedures unless competency and Physician approval is documented. Job Description Essential Responsibilities: 1. Operates with a high level of autonomy through use of excellent critical thinking and problem solving skills as applicable to patients in the acute care setting and clinic setting. a. Conducts appropriately detailed assessments/evaluations of patients and families which may include health histories, physical examinations and evaluation of health risk factors; b. Orders and interprets lab, radiographic, bedside records, and other clinical data sources in the context of the patients diagnosis(ses) and condition; c. Establishes medical diagnosis(ses); d. Orders and evaluates diagnostic studies; e. Performs and monitors therapeutic procedures; f. Documents historical, physical, assessment and plan information to the medical record. 2. In conjunction with the Physician, nursing and other health care disciplines, the APP is responsible to evaluate and help direct the individualized care of patients. 3. Prescribes and regulates medications per written physician practice agreement and in compliance with prescriptive authority requirements. 4. Performs and documents results of History and Physical examination for admission, observation, consultation or clinic visit for assigned patients. 5. Performs daily, pertinent bedside evaluation and records appropriate progress note for physician review and authentication. (Inpatient only) 6. Prepares discharge paperwork, including discharge summary, for assigned patients on the day prior to or day of discharge. At the ICC/OM, discharge summary completed prior to patient discharge. 7. Writes admitting, daily, and discharge orders appropriate to the patient’s condition and plan of care. (Inpatient only) 8. Works collaboratively with Physician(s) in an environment that engenders teamwork to be certain that care is rendered efficiently. 9. Communicates regularly and effectively with patients and their families. 10. Communicates regularly and effectively with referring physicians and support staff (such as nursing, rehabilitative services, etc.) 11. At least daily and as needed for appropriate clinical input, meets with Physician(s) to review results of evaluation and planned care. 12. Supports and participates in focused quality, safety and utilization studies as requested to improve quality and cost-effectiveness of patient care. 13. Alerts Physician(s) to acute declines in patient status, critical lab values and/or radiographic findings to review planned treatment or request bedside evaluation by Physician. 14. Complies with applicable medical staff rules and regulations and health information management policies to support an accurate and complete medical record. 15. Prepares required billing materials and submits to billing in a timely manner. 16. Shares in on-call rotation with physician back-up available. (Not applicable at ICC/OM) 17.
