JOB SUMMARY Responsible for the development, implementation, monitoring and supervision of the restorative nursing program for the facility. Acts as the liaison between formalized therapy and nursing. Promotes a restorative nursing philosophy among all facility departments and work closely with the Ladies and Gentlemen of Alden to ensure the restorative plan of care is being followed thereby, promoting customers to their highest level of function. QUALIFICATIONS: Must possess a current, unencumbered license to practice as an RN or an LPN in the state of Illinois. Direct charting in his/her shift and make monthly detailed evaluation of all resident charting so that charts reflect progress and condition of residents in the EMR system. Be responsible for well-being and nursing care of all residents assigned to his/her unit while on duty. Contact families regarding significant decline in resident's status or transfer to hospital documenting contact in the EMR. Make rounds and observe individual residents who are experiencing episodes of acute illness, deterioration in health status, recent injury, recovering from surgery, etc., so as to be acquainted with the resident's status, both physically and emotionally and to ascertain that the staff is rendering proper care. At all times abide by policies of the facility and ascertain that employees under his/her supervision do the same. Supervise serving of prescribed diets and fluid intake (it is acceptable to document only deviations from normal) and report persistent unresolved problems to the DON. Ensure that all medications and treatments are charted by the person administering the medication or completing the treatment on his/her assigned shift (EMR system). Prepare incident/accident reports, events and observations using the EMR system. Ensure that a copy of the medical records or an abstract of them are forwarded with the transfer of residents to a hospital or other health care facility. Detect and correct situations that have a high probability of causing accidents or injuries to residents and/or staff. Assure that work/assignment areas are clean and free of hazardous conditions, i.e. spills, excess carts, etc. and that equipment, tools, supplies, etc. are properly stored while working, as well as before leaving such areas for breaks, meal times, and end of the work day. Assure that established infection control and universal precaution practices are maintained when performing nursing procedures. Supervise the employee status verification in the State Registry upon application as well as the criminal background check when necessary. Overview Provides compassionate, culturally sensitive, holistic nursing care to patients in a clinical setting, supporting the Mercyhealth nursing mission, practice values and strategic plan. May include variable shifts, weekends, holiday and/or on-call hours. Performs other duties as assigned including floating to various units based on identified needs and clinical competency. Responsibilities ESSENTIAL DUTIES AND RESPONSIBILITIES · Utilizes the nursing process and collaborates with interdisciplinary team members in caring for patients. · Administers prescribed medications and treatments in accordance with approved nursing techniques, policies and procedures, standards of care, and State laws practice act. · Documents according to standards of care. · Performs assessments, planning, intervention, and evaluation of care. · Develops, initiates, and executes a patient plan of care according to the individualized needs of the patient, as prescribed by Physician and or clinic policy including patient and family instruction. · Participates in department or unit performance improvement activities, may include shared governance. · Utilizes clinical expertise and guidance to other units’ departments in support of the patient’s longitudinal plan of care. · Utilizes critical thinking regarding patient safety and patient experience and appropriately escalates to the appropriate chain of command. · Participates in the professional nursing continuum process, unit practice activities and decisions and peer review process. EDUCATION and EXPERIENCE Graduate of an accredited nursing program. ADN required upon hire, BSN preferred. For example : ACLS, PALS, PEARS, and NRP ADDITIONAL REQUIREMENTS Passing the Driver’s License Check and or Credit Check (for those positions requiring).
