Job Responsibilities

Delivering post-acute care as an LPN
Monitoring the physical and mental wellbeing of residents and tracking their vital signs
Performing intakes of residents and documenting their health concerns and medical histories
Reviewing the medical records and charts of residents
Properly administering (and verifying) medications
Helping to maintain appropriate levels of medical supplies
Contributing to a safe, comfortable environment for residents
Assisting with IV set up, vaccinations, and phlebotomies
Being accountable to follow and adhere to care plans for residents
Helping medical staff with orientation and training

Qualifications and Skills

Current Illinois state license to practice as a Licensed Practical Nurse (LPN)
Completion of approved Licensed Practical Nurse (LPN) educational program
High school diploma (or GED/equivalent)

Assist in developing a written plan of care for each customer that identifies the problems/needs of the customer and be able to evaluate the delivery of care. Attends and actively participate in customer care plan conference. Consult with other nurses, management, and other related health professionals to assist in assessing, planning, and delivering and evaluating patient care. Plan, conduct and participate in In-service Education Programs, workshops, seminars, conferences, and courses. Make written and oral reports including recommendations concerning the wound care program. Ensure that the wound assessment reflects that the care plan is being followed. Participate in the development, maintenance, and implementation of the facility’s Quality Assurance program for the Wound Care Program. Coordinate facility wound audits with the nursing staff.
