Policy Development: Develops and implements processes, policies, and procedures, coordinating and overseeing all patient services provided by clinical personnel. Operational Monitoring: Interprets operational indicators to monitor census changes and adjusts staffing levels, revenues, or expenses accordingly. Compliance: Ensures compliance with the Granite Peak Home Health Employee Handbook and policies. HR Functions: Participate in the hiring, evaluation, and termination of organization personnel. Orientation and Training: Develops, implements, and evaluates orientation for new Granite Peak Home Health personnel and plans in-service and continuing education programs. Performance Evaluation: Conducts quarterly conversations and annual performance evaluations of supervised staff. Record Maintenance: Collaborates to maintain accurate patient records and monitors statistics and reports for evaluation and reporting. Supply Management: Ensures adequate and appropriate supplies and equipment for patient services. Quality Assurance: Assists with evaluating performance via the QAPI program, ensuring staff productivity and safe, quality home health services. Goal Development: Participates in developing goals and administers Granite Peak Home Health policies and procedures. Regulatory Compliance: Collaborates with the HR department to ensure compliance with local, state, and federal laws regarding licensure and certification. Industry Awareness: Stays informed about nursing and home health care changes, adjusting procedures accordingly. Regulatory Knowledge: Remains updated and familiar with the rules and regulations of third-party agencies, state licensure, and Medicare standards. Referral Promotion: Promotes referrals to Granite Peak Home Health through community awareness, marketing plans, and building relationships within the communities Granite Peak Home Health serves. Staff Development: Identifies training needs and opportunities for professional development among staff. Creates and implements development plans to enhance staff skills and competencies. Mentors and supports staff in achieving their professional goals. Organizes workshops, seminars, and other educational activities to foster continuous learning. Performance Metrics: Establishes and monitors key performance indicators (KPIs) for clinical staff to ensure high-quality patient care. Tracks and analyzes patient outcomes, satisfaction scores, and other relevant metrics to identify areas for improvement. Reports on performance metrics to senior management and uses data to drive decision-making and process improvements. Ensures compliance with all regulatory and accreditation standards through regular audits and reviews. Effectively communicates ongoing with patient and family progression/changes in POC. Effectively communicates with Supervisor/Nursing Director/other disciplines in the case. Communication with the patient’s MD (verbally and/or in writing) to obtain effective treatment modalities and/or rehabilitative therapy modalities. Actively communicates in the case conferencing sessions to establish best practices. Submits requests for re-authorization of “more visits needed” to payers timely. Coordinates Community Services for the patient to assist in safe home care needs. Participates in the Performance Improvement Committee process.
