NFP Ambulatory RN - Supervisor

This role is accountable for providing and coordinating competent, evidence-based care across the outpatient setting, including assessment, diagnosis, planning, implementation, and evaluation. The supervisor supervises a multidisciplinary team of maternal-child home-visiting nurses across six counties, supporting 200 families with children from early pregnancy to age two. Overview Location: Bloomington Hospital Position: Clinical Nurse - Ambulatory Navigator Shift: Monday-Friday Day shift The Clinical Nurse - Ambulatory Navigator coordinates the care needs of assigned patients and develops comprehensive plans to manage care delivery across the patient care continuum. Partners with patients and their primary physicians to develop customized care plans based on individual needs and preferences. Collaborate with physicians, nurses, health professionals, social workers, and others to ensure appropriate tests and treatments are delivered in a timely fashion. Position Overview: As a vital member of IU Health Medical Group’s statewide quality department, the Clinical Nurse-Ambulatory Quality Coordinator will support our Specialty Care practices by driving quality improvement initiatives, managing key metrics, and providing comprehensive data reporting. This role involves training providers and clinical support staff on quality tools, metric specifications, and workflows to effectively close care gaps. The position requires proficiency with quality reporting systems, dashboards, and routine performance analysis, with regular communication and collaboration with practices, outreach coordinators, and leadership at all levels.
