Job Description​ We are looking for Advanced Practice Providers who will be floating between St. Luke's East and Perryman location. This is a full time position Monday-Friday, 7:00 am-5:00 pm. No weekend and holiday rotations. 2 years of APP experience required. Key Responsibilities • Provide timely care for patients presenting with acute, non-life-threatening conditions in a same-day clinic setting. Acute needs: Examples: URI, bronchitis, otitis, rash, UTI, sprains/strains Urgent needs: Examples: acute abdominal pain w/o severe symptoms, injuries, migraines, nausea/vomiting, blood pressure elevation Med Refill visit if visit with Care Team is not available in timely fashion. Sports physicals Annual Wellness Visits if needed based on calendar constraints. Collaborate with clinical teams to triage patients and determine the urgency and type of care required. Collaborate with advanced practice providers, nurses, and other healthcare team members to ensure continuity and quality of care. Refer patients to specialty care, primary care follow-ups, or emergency services as appropriate. Close care gaps (tee up labs for open lab gaps and help reschedule patient with PCP) Close HCC gaps Help set up follow up appointments with PCP for needed care. Work closely with the care team and scheduling to help direct patients to the most appropriate level of care. Caregiver Responsibilities: Provide non-medical support and companionship to senior clients Perform personal care duties; such as bathing, dressing, grooming, and toileting Issue medication reminders and maintain medication schedule Conduct grocery shopping, meal preparation, and light housekeeping duties Support the client's mobility needs by assisting with walking and transfers Complete household errands and provide client with transportation to doctor's appointments Job Requirements: Ability to pass state/nationwide background check and drug screening Active phone where you can be reached, for scheduling purposes Minimum of 2 years of caregiver experience preferred Current Driver's license is preferred This position requires travel to client homes in Springfield, MO and surrounding communities.
