The program prioritizes individuals experiencing homelessness, serious mental illness (SMI), co-occurring disorders (COD), and hepatitis C (HCV)—populations often underserved in traditional care models. This initiative integrates same-day HIV/HCV testing and diagnosis with immediate access to behavioral health services and aims to improve outcomes in high-need communities while developing scalable models for HCV elimination. The Nurse Care Manager (RN) serves as the primary clinical liaison between clients, Peers, and healthcare providers within the PATHS-CC program. As a licensed Registered Nurse, the RN Care Manager is responsible for conducting telemedicine-based clinical screenings, coordinating care, triaging clients to appropriate same-day telemedicine visits, and managing follow-up activities to ensure timely and effective treatment across a range of conditions. The RN works closely with Site Peer Coordinators and Peers to support clients through case management, facilitated referrals, and system navigation. Department Overview Provides support to the nursing team for Doernbecher PeriAnesthesia Services. The CNA assists in the care of patients in the PeriAnesthesia areas, by performing tasks within their scope of practice and maintaining a safe, clean, efficiently stocked environment. Doernbecher PeriAnesthesia services provides inpatient and outpatient pre and post care to pediatric patient’s birth to 18 years of age and all conditions undergoing procedures. Services extend to adult patients for congenital conditions under the care of a pediatric provider. DCH Perioperative Services is a department within Perioperative Services within the Nursing Division. Function/Duties of Position Assist with the admitting and discharge Obtain accurate vital signs and weight Complete all unit specific CNA documentation in the patient’s medical record related to: admissions, daily care, vital signs, weight, intake and output, discharge, etc. Assist the patient with any and all basic hygiene, lifting, transport, dietary support, and other personal needs and medical necessities Support patient and family centered care Report potential compliance problems as outlined in the Code of Conduct Model ethical and appropriate behavior, professional standards, compliance, and personal integrity Must be able to perform the essential functions of the position with or without accommodation Required Qualifications High School Diploma or GED Oregon CNA1 Certification at time of hire BLS required at time of hire, issued by the American Heart Association (AHA) Additional Details Benefits: Two raises per year – One at anniversary date and one across the board annual increase Healthcare Options - Covered 100% for full-time employees and 88% for dependents, and $25K of term life insurance provided at no cost to the employee Night Shift Differential Two separate above market pension plans to choose from Vacation- up to 200 hours per year depending on length of service Sick Leave- up to 96 hours per year 8 paid holidays per year Substantial Tri-met and C-Tran discounts Additional Programs including: Tuition Reimbursement and Employee Assistance Program (EAP) All are welcome Oregon Health & Science University values a diverse and culturally competent workforce.
