Summary
Responsible for nursing care of assigned residents, under the direction of a Registered Nurse; to ensure the goal of providing care to our residents in an atmosphere of comfort, independence, and dignity.
Essential Functions
Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
Supervises unit care of residents, assigns duties to Certified Nursing Assistants, evaluates completion of duties, and makes reassignments as necessary to ensure provision of service to the residents.
Observes, documents and reports any significant changes in the resident's condition that warrants medical and/or nursing intervention.
Assures the provision of nursing services and takes action to ensure fulfilling resident wishes and promoting the least restrictive alternative of care.
Revises care plans daily as needed, by identifying problems, formulating measurable goals, and incorporating resident/family wishes and teaching; while respecting and promoting the resident to self-direct their own care.
Administers medications to assigned residents as ordered by the physician, including intravenous services.
Required Skills/Abilities
Consistent execution of quality customer service. Maintains and demonstrates ethical conduct. Able to effectively communicate with geriatric and/or confused residents and a variety of other people such as family members, visitors, vendors, Private Duty Residents, and staff. Positive and constructive attitude. A working knowledge of nursing theory, proficient in nursing procedures and infection control practices. Ability or skill in judgment, decision-making, communication, organization, supervision, documentation and follow through. Ability to identify reactions to medications and treatments. Demonstrated computer proficiency. Experience with Electronic Medical Records (EMR) preferred.
Working Environment & Physical Demands
This position is required to regularly work on campus.
Works in a well-lit and well-ventilated facility. May be exposed to confused, belligerent or physically aggressive residents, may be exposed to contagious diseases and mental stress. Subject to frequent interruptions. Manual dexterity, good vision and hearing is required. Must be able to read product labels and distinguish colors. May be exposed to a variety of pharmaceutical products, cleaning agents and their fumes. May be exposed to bodily fluids.
Required Education, Experience, Licenses, and Certifications
Graduate of an accredited School of Practical Nursing required.
Current, unencumbered license in the state of Oregon as a Licensed Practical Nurse required.
