As a Wound Care Nurse LPN, you will provide each resident with routine daily nursing care and services in accordance with each resident’s assessments and care plan and as may be directed by management. This distinctive role will specialize in the care of patients with wounds, ostomies, or continence disorders. Wound Care Nurse LPN Duties and Responsibilities: Works with patients who have gastrointestinal stomas for feeding tubes or colostomy bags, skin ulcers, drains, surgical incisions, or pressure sores. Evaluates & Provides treatment protocols to ward off skin irritation and infection for wounds that may not heal well, because of other medical problems, such as diabetes or poor circulation. Performs administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary. Implements standards of nursing practice in infection prevention reviewing and revising as necessary to reflect changes in nursing practice and resident population. Assists with the provision of nursing services 24 hours a day, seven (7) days a week. Provides licensed care to assigned residents as ordered by physician and in accordance with facility, federal, state and local standards, guidelines and regulations Charts nurses notes in an informative and descriptive manner that reflects the care provided as well as the resident’s response to the care. Monitors and directs non-licensed ancillary services as deemed necessary to meet professional standards and maintain the highest practicable level of resident care Communicate/report information to residents, resident families, physicians, guardians, governmental agencies, and facility personnel as required by facility policy and State/Federal guidelines. Follows and adheres to company policies and procedures regarding HIPAA (Health Insurance Portability and Accountability) compliance. All other duties, as assigned. Wound Care Nurse LPN Qualifications and Skills: Excellent written and verbal communication skills in English. Serves as the expert in Wound Care. Ability to multi-task and remain calm under stress. Analytical and problem-solving skills. Understands and follows infection control standards, policies and procedures. Must be able to adapt to a flexible work schedule and occasional overnight travel. Wound Care Nurse LPN Education and Experience: Nursing Degree or diploma from an accredited College or University. Possess a current license to practice in the State as a Licensed Practical Nurse. 1 year of experience preferred. Long-Term care experience preferred. Physical Requirements: Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, ability to distinguish smells and temperatures, all with or without the aid of mechanical devices is required. Limited potential for exposure to environmental hazards. Understanding and adherence to company safety standards and protocols required. The Wound Care Nurse (LPN or RN ) is responsible for assessing, treating, and monitoring residents with wounds, skin conditions, and pressure injuries. This role ensures evidence-based wound care practices, regulatory compliance, and collaboration with the interdisciplinary care team to promote healing and prevent complications. Essential Duties & Responsibilities Perform comprehensive wound assessments and ongoing evaluations Develop, implement, and update individualized wound care treatment plans Provide hands-on wound care, including dressing changes, debridement (as allowed by license), and skin treatments Accurately document wound progress, measurements, photos, and treatments per facility policy Monitor residents for signs of infection, delayed healing, or complications and report changes promptly Collaborate with physicians, nurse practitioners, dietitians, therapy, and nursing staff Educate residents, families, and staff on wound prevention, treatment, and skin integrity Ensure compliance with CMS, state, and facility regulations related to wound care Participate in Quality Assurance and Performance Improvement (QAPI) initiatives Assist with pressure injury prevention programs and audits Maintain infection control standards and proper use of supplies Participate in care plan meetings as needed Qualifications Current Nevada LPN or RN license in good standing Minimum 1 year of nursing experience ; SNF or long-term care preferred Wound care experience strongly preferred Wound Care Certification (WCC, WCN, or similar) preferred Knowledge of pressure injury staging, treatment protocols, and skin integrity best practices Strong assessment, documentation, and communication skills Ability to work independently and as part of an interdisciplinary team
