LPN or RN Wound Nurse

Working Conditions Works in office areas as well as throughout the nursing services area (i.e., drug rooms, nurses’ stations, resident rooms, etc.). Moves intermittently during working hours. Is subject to frequent interruptions. Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances. Is subject to hostile and emotionally upset residents, family members, personnel and visitors. Communicates with the medical staff, nursing personnel and other department directors. Works beyond normal working hours and on weekends and holidays when necessary. Is subject to call back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.). Is involved in community/civic health matters/projects as appropriate. Attends and participates in continuing education programs. Nursing and Medical Services Is subject to injury from falls, burns from equipment, odors, etc., throughout the workday as well as to reactions from dust, disinfectants, tobacco smoke and other air contaminants. Is subject to exposure to infectious waste, diseases, conditions, etc., including COVID-19, tuberculosis and the AIDS and Hepatitis B viruses. May be subject to the handling of and exposure to hazardous chemicals. Maintains a liaison with the residents, their families, support departments, etc., to adequately plan for the residents’ needs. Education Must possess, as a minimum, a degree in nursing from an accredited college or university or be a graduate of an approved registered nursing program. Experience Must have, as a minimum, two (2) year(s) of experience as a registered nurse. Specific Requirements Must possess a current unencumbered active license to practice as a registered nurse in the state of practice. Must be able to read, write, speak and understand the English language. Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served. Must be a supportive team member, contribute to and be an example of team work and team concept. Must possess the ability to make independent decisions when circumstances warrant such action. Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public. Must be knowledgeable of nursing practices and procedures as well as laws, regulations, and guidelines that pertain to long-term care. Must possess the ability to plan, organize, develop, implement and interpret the programs, goals, objectives, policies, procedures, etc., that are necessary for providing quality care. Must have patience, tact, a cheerful disposition and enthusiasm as well as the willingness to handle difficult residents. Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices. Must be able to communicate information concerning a resident’s condition. Must not pose a direct threat to the health or safety of other individuals in the workplace.

LPN 12 Hour Shift

Provide routine care, observe residents’ health, assist doctors, and communicate instructions to residents regarding medication, home-based care, and preventative lifestyle changes. Job Responsibilities: Help implement and evaluate care plans for individual care of each resident. Maintains appropriate nursing service objectives, goals, and standards of nursing practices. Receive, transcribe, and implement physician’s orders in a timely manner. Reports required condition changes, events to appropriate parties. Administers medication/treatments as prescribed. Monitors infection control compliance. Investigates/documents accidents/incidents. Record a resident’s medical history accurately Take and record measurements of blood pressure, temperature, heart rate etc. Observe residents under treatment to identify progress, side-effects of medications etc. Monitor residents’ condition including fluid intake and output and compose residents' charts Assist ailing residents in daily necessary activities such as eating, bathing etc. Administer injections, prescribed medications, enemas etc., clean and dress wounds and assist with other basic medical care tasks Collect samples for lab testing as assigned Provide emotional and psychological support when needed Communicate with residents’ family or friends to provide advice, comfort and release instructions Job requirements: Proven experience as LPN or similar role Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team Familiarity with hospital health, safety and sanitation standards and procedures Understanding of confidentiality obligations and nursing best practices Excellent knowledge of medical and hospital terminology Good knowledge of MS Office and data entry Perfect physical condition and stamina Excellent communication and interpersonal skills Compassionate and able to handle stress Successful completion of LPN license program is a must

Director of Nursing

The primary purpose of this position as the Director of Nursing in planning, organizing, developing and directing the day-to-day functions of the nursing services department in accordance with current federal, state and local standards, guidelines and regulations that govern the facility. Assist in developing and implementing a nursing services organizational structure. Work with the Minimum Data Set/Resident Assessment Instrument (MDS/RAI) Coordinator to schedule, conduct and submit resident assessments. Allocate resources efficiently while ensuring that residents obtain their highest practical physical, mental and psychosocial well-being. Collaborate with medical professionals, consultants and organizations including government and advocacy groups. Coordinate with other resident services to ensure continuity of resident care. Read and interpret long-term care regulations and requirements of participation. Assist in the development of nursing policies and procedures which meet the current standards of nursing practice, comply with state and federal regulations, reflect facility philosophy and support operational objectives. Assist in obtaining staffing information that must be posted on a daily basis. Participate in developing, implementing and periodically updating the resident’s written discharge plan. Develop and periodically update nursing services objectives and statements of philosophy. Ensure that reference material (i.e., Physicians’ Desk Reference (PDR), regulations, professional standards of practice, etc.) maintained at the nurses’ stations is current; recommend to the Director of Nursing written material that will assist the nursing services department in meeting the day-to-day needs of residents. Monitor the facility’s quality measure reports.
