Reports abnormal findings immediately to physician. · Responds to Diagnostic Data and initiates appropriate action or intervention. · Assesses patient education needs upon admission and daily thereafter. · Assesses psychosocial needs specific to patient’s age, growth and development. · Establishes priorities for patient care based on patient needs, evidenced by the completion of assigned duties within shift. · Verbalizes understanding of patient diagnosis and interdisciplinary plan of care. · Safely and correctly initiates and documents nursing interventions in emergencies. · Administers, observes, and documents intravenous therapy according to hospital policy. · Administers medications within 30 minutes of scheduled time, observes and documents effectiveness, recognizes and reports adverse reactions immediately. · Provides individualized instructions each shift to patient/significant other regarding the plan of care. · Demonstrates safe and correct use of unit specific equipment. · Ensures that each patient’s general hygiene needs are met daily. · Performs pre-operative and post-operative care and instructions for each assigned surgical patient during shift. · Adheres to standard precautions including hand hygiene guidelines at all times. · Documents specific patient valuables and arranges appropriate storage. · Performs dressing changes using aseptic technique. · Collects and labels specimens per policy when ordered by physician. · Performs bedside glucose monitoring according to policy. · Administers internal feeding via ng tube, gastrostomy tube and/or jejunostomy tube. · Performs ostomy care to include irrigation, appliance usage and reinforcement of teaching · Inserts intermittent or indwelling urinary catheter using aseptic technique. · Administers oxygen therapy according to physician order. · Documents/reports patient responses or outcomes. · Evaluates and documents patient’s progress towards established goals. · All other tasks as related to the positions duties. Demonstrates a calm, professional manner during a crisis or emergency situation at all times within the patient care environment. · Provides all patient care respectfully demonstrating/reviewing patient rights utilizing the nursing process and documenting in the acceptable hospital charting format. · Maintains patient care equipment, alerts chain of command about necessary repairs, "red tags" broken equipment, and initiates calls to engineering/biomedical engineering as needed · Initiates admission data base and MAR for outpatient and AM admit surgeries. · Verifies correct patient by using current photo identification · Educates/reviews pre-operative instructions with patient/family prior to day of surgery for AM admit and outpatient surgeries. · Initiates pre-operative medication instructions per surgeon/anesthesia orders. · Initiates Perioperative and Perianesthesia patient/family education. · Implements all new physician orders within two hours of initiation for routing and within 30 minutes or less for stats. · Performs initial vital signs and admission history to include introduction to patient and family. Notification of anesthesia team of patients arrival to pre-testing area. · Obtains and documents a complete nursing admission assessment, including physiological and psychological factors to develop a plan of care for outpatient and AM admit surgeries. · Performs and documents an accurate systems assessment. · Assess availability of transportation home after an outpatient procedure. Educates patient/family regarding the need for transportation home with a responsible adult after an outpatient procedure. · All other duties as assigned. Complete comprehensive nursing admission assessment within eight hours, including physiological, psychological, and psychosocial factors. Conduct and document system assessments at the beginning of each shift. Assess and respond to diagnostic data, patient education needs, and psychosocial requirements. Operate and maintain unit-specific equipment, including aseptic techniques for dressing changes and catheter insertions. Follow standard precautions, hand hygiene, and hospital protocols for intravenous therapy, specimen collection, and glucose monitoring. Implement skin and wound protocols, perform dressing changes, and provide ostomy care as required. The incumbent conducts initial, concurrent and retrospective chart reviews for clinical utilization and authorization. The Utilization Review Case Manager coordinates with the healthcare team for optimal and efficient patient outcomes, while avoiding treatment delays and authorization denials.
