If you enjoy autonomy, working with geriatrics, and controlling your schedule, this position may be for you! This is a PRN position with PTO and FLEXIBLE HOURS! We are looking for a provider that can dedicate 4-5 days a week. Increased salary also available with productivity and benefits to be discussed upon interview. Cincinnati/Dayton area is growing, and this position could become full-time. PURPOSE According to prescribed policies and procedures of the organization including all applicable state, federal and accreditation regulations and under the general supervision of the Program Manager, the Nurse Practitioner is responsible for providing nursing care services to residents/clients as assigned, making observations on an advanced level, forming nursing judgments, carrying out nursing actions based upon understanding of the scientific principles involved, reporting and recording observations appropriately and supervising staff. Qualified candidates for this position offer: Education : Graduate of an accredited school of nursing with a Master’s Degree in Nursing and a nurse practitioner certification. Experience : Minimum of two years as an RN in a health care setting. Licensure : Current Ohio RN License and Nurse Practitioner in good standing. ESSENTIAL FUNCTIONS Conducts physical exams and comprehensive medical and social history of individuals, including those who are healthy and those with acute illnesses and chronic disease. Makes independent decisions regarding management and treatment of medical problems identified Orders, performs, and interprets laboratory tests for screening and for diagnosing. Prescribes and orders appropriate diet, medications and other forms of treatment such as physical therapy. Performs therapeutic or corrective measures, including urgent care. Refers individuals to appropriate specialist nurses or physicians or other health care providers. Provides information, instruction, and counseling on health maintenance, health promotion, social problems, illness prevention, illness management and medication use. Evaluates the effectiveness of instruction and counseling and provides additional instruction and counseling as necessary. Initiates and maintains follow-up of noncompliant patients. Serves as preceptor for medical, nursing, NP, or physician assistant (PA) students. Consults with other health care providers about established clients who have been admitted to hospital, home car, rehabilitation, or nursing homes. Corresponds with insurers, employers, government agencies, and other health care providers about established clients as necessary. Manages care of clients; develops plan of treatment and/or follow-up and monitors progress, determines when referral to another provider is necessary, makes necessary arrangements for further care, determines when hospital admission or emergency room visit is necessary, and determines when illness is resolved. Assesses social/economic factors for each client, and tailors care to those factors. Maintains familiarity with community resources and connects clients with appropriate resources. Contracts with clients regarding provider responsibilities and client responsibilities. Maintains an acceptable level of productivity to meet the facility goals. Position Details: Work Shift: Day Shift - 10 (United States of America) Scheduled Weekly Hours: 40 Department: Internal Medicine GPCP Overview of Position: The advanced nurse practitioner will provide medical services in a primary care clinic in collaboration with family medicine or internal medicine physicians. The nurse practitioner will provide general and preventative care, conduct check-ups, treat illnesses, order lab tests, and prescribe medication for children and adults.;;The nurse practitioner will be responsible for assessing, planning, implementing, evaluating, and documenting patient care, frequently advising patients on health maintenance and disease prevention or providing case management.; ESSENTIAL DUTIES 1. Conducts thorough medical histories, performs complete physical examinations (where indicated), initiates appropriate lab, radiology tests or other specialized tests required for evaluation of illness, and scrutinizes lab data on new and former clinic patients to establish a record of the patient's current health status and to develop a working diagnosis and treatment plan. 2. Provide primary care services on the days and at times established and scheduled by Genesis Healthcare System. 3. Must be able to work with collaborating physicians when applicable. 4. Performs routine outpatient procedures such as injections, immunizations, suturing and wound care, managing simple conditions produced by infection or trauma, assisting in the management of more complex illness and injury as privileged by the Genesis Credentialing Board. 5. May prescribe and monitor medications as allowed by prescriptive authority through applicable state Board of Nursing/Medicine and Genesis Credentialing Board. 6. Ensures continuity of care by serving as a liaison between patient and other members of the multi-disciplinary care team or with other specialty areas as necessary. 7. Participates in quality monitoring through the review of records and treatment plans for patient outcomes periodically to assure quality care. 8. Maintain competence in his/her area of specialty by attending professional conventions or seminars as well as participating in educational activities offered by professional societies on a reasonable basis 9. Other duties as assigned. QUALIFICATIONS 1. APRN: EDUCATION: Graduate of an accredited APRN training program. 2. Current Ohio RN and APRN licensure 3. Must have a current certificate of authority and certificate to prescribe (or eligible) by the Ohio Board of Nursing. 4. Family Medicine or Internal Medicine experience preferred. 5. Required Certifications: BLS to be obtained within 6 months of hire. ACLS or PALS may be required based upon practice setting. PATIENT CENTERED CARE & BEHAVIORAL EXPECTATIONS 1. We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities. WOUND NP ROLE AND RESPONSIBILITIES: Provide wound care services for long-term care patients residing in skilled nursing facilities. Prescribe appropriate medications, including controlled substances in accordance with OH Law. Order, review, and/or follow-up with treatments and diagnostic testing. Communicate with the nursing home staff on a regular and as needed to discuss changes in condition. and/or the care plan. Collaborate with the primary care physician. WOUND NP QUALIFICATIONS AND EDUCATION REQUIREMENTS Current, unrestricted NP license in the state of Ohio. Board Certification. DEA licensure (Optional) Clinical background in adult, family, or geriatric specialties. Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers. Must be comfortable staging pressure injuries according to the NPIAP Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc. Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices Strong preference to having wound certification or be open to getting certified Experience working in a long-term care setting. Ability to work with an EMR. Ability to work within an interdisciplinary care team to coordinate patient care.
