Transitions of Care RN Manager

Role Overview We are currently seeking a TOC RN Manager to lead and support a team of RN Case Managers who deliver exceptional care and patient outcomes. This leader will help shape culture, performance, and clinical excellence by mentoring nurses, supporting their clinical decision-making, and ensuring care is delivered with compassion, integrity, and skill. As part of our interdisciplinary leadership team—alongside peer RN Managers and leaders from Nurse Practitioner, Care Coordinator, and Associate disciplines—you’ll collaborate on day-to-day operations, patient care strategy, and the continuous improvement of our virtual care model. Job Description A bit about this role: This is an exciting opportunity for a compassionate and skilled Advanced Practice Provider (APP) to provide virtual primary care through our innovative telehealth platform. As a Virtual Primary Care Provider you will deliver comprehensive, high-quality care while fostering strong, lasting patient relationships. You will provide ongoing management of chronic conditions, preventive screenings, acute care visits, and medication management, all through a virtual care model. You'll also work collaboratively with a multidisciplinary team to coordinate in-person services such as lab work, imaging, and specialist referrals as needed. Bilingual (Spanish/English) Desired skills and experience: Experience managing chronic conditions, preventive care, and acute illnesses via telehealth. Familiarity with value-based care models, population health management, and quality metrics (HEDIS, STAR ratings, etc.) is a plus. Comfortable with digital documentation and telemedicine platforms (experience with EHRs and virtual care workflows preferred). Salary range: $115,000 - $150,000 annually The pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting. The actual base salary offered will depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
