Collaborates with a physician(s) and other clinicians in providing a full scope of patient care. The focus is on preventive health or the management of acute and/or stable chronic conditions. Performs medical services that have been specifically authorized and directed by the credentialing body and the certifying organization through which they hold licensure. Position Summary The APP provides health care services to patients working in collaboration with a licensed physician member of the medical staff. Minimum Requirements: Active and unrestricted Psychiatric Mental Health Nurse Practitioner license in the state of Ohio. Board Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP) . Active DEA license . Minimum of one year of psychiatric experience required; geriatric experience preferred. Strong interpersonal and communication skills. Must be willing and able to travel locally up to 1 to 1.5 hours to assigned facilities. We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities. NP ROLE AND RESPONSIBILITIES: Provide primary and preventative care for long term care patients residing in skilled nursing facilities. Manage Residents with acute change in condition. Advanced care planning and chronic care management. Prescribe appropriate medications, including controlled substances in accordance with OH Law. Order, review and/or follow-up with treatments and diagnostic testing. Communicate with the nursing home staff on a regular and as needed to discuss changes in condition. and/or the care plan. Collaborate with primary care physician. NP QUALIFICATIONS AND EDUCATION REQUIREMENTS Current, unrestricted NP license in the state of Ohio. Board Certification. DEA licensure.
