Nurse fellows join an interprofessional class, the Harvard Interprofessional Palliative Care Fellowship, of nurse practitioners, physicians, social worker, and pharmacist who share the goal of a career in palliative care. Job Summary Reporting to the Director of Hospital Medicine, provides daily evaluation and management to inpatients. This work will be in collaboration with the hospitalist service physician or surgical attending physician, and any physician specialists involved with each patient. The supervising physician of record will be one of the hospitalist physicians for purposes of establishing a collaboration agreement for licensure and prescribing status as well as periodic performance reviews. A second supervising physician may be established for surgical cases. Day-to-day nurse practitioner patient care supervision may be delegated to any one of the on-site hospitalist physicians or surgeons, as applicable. The degree of autonomy of the nurse practitioner’s daily practice will depend on the details of each particular case and the preferences and skills of the NP. Boston Health Care for the Homeless Program is looking for a Nurse Practitioner/ Physician Assistant who will provide comprehensive, high-quality primary, episodic and/or recuperative care at our primary care clinic and respite care facility. It is a collaborative practice where medical, behavioral health, and case management staff work to meet the various needs of patients during their stay. Responsibilities: Provide comprehensive, high quality primary or episodic care at various clinical sites staffed by Boston Health Care for the Homeless Program; work as part of multi-disciplinary team in caring for patients May work at shelter-based clinic, hospital-based clinic, respite facility or at one of of primary care clinics Elicit an appropriate history, perform physical examinations, order diagnostic studies, interpret results and initiate therapy in accordance with the Guidelines for Physician Assistants and Nurse Practitioners of Boston Health Care for the Homeless Program Refer patients as appropriate to medical specialists, hospital admission, recuperative care and detox facilities, providing appropriate information for continuity of care; provide education to patients on health promotion and disease treatment and prevention Participate in collaborative relationship with BHCHP physician according to the Regulations of the Commonwealth of Massachusetts Maintain adequate, up-to-date medical records, completed in a timely manner, according to the guidelines of BHCHP, employing standard forms such as problem lists, medication lists, etc. Maintain active DPH, DEA, and professional license, hospital privileges, managed care credentialing and CPR training. Qualifications: Licensed by the Commonwealth of Massachusetts Registered or eligible to be registered with the Massachusetts Department of Public Health and the Federal Drug Enforcement Agency At least one year of NP/PA experience required Experience in working with underserved populations, mentally ill, homeless and/or persons with substance use disorders preferred Life support training certification (e.g. CPR, BLS, ACLS) from a course that includes a hands-on, in-person component Bilingual proficiency preferred Compensation: The compensation is $77.00 per hour.
