Our Concord Family Medicine office provides physical examinations, routine health screenings, and preventative care for patients of all ages. The goal is to identify and control patients' health risk factors before they become problems. We diagnose and treat acute and chronic illnesses, and promote general health, disease prevention and overall well-being.
We are seeking an experienced, full-time 40-hour Nurse Practitioner to support our office located at 80 Beharrell Street, Concord MA, 01742. We are open to our patients Monday-Friday from 8:00 AM to 5:00 PM. The NP’s weekly schedule will consist of 32 hours of clinical, patient-facing time and 8 hours of administrative time. The NP will be able to select their preferred administrative day (option to choose any day except for Monday or Friday). The administrative day can also be remote.

The NP will also carry their own panel of approximately 900 patients to enhance practice operations, increasing patient volume and efficiency while providing excellent patient care and experience. This includes traditional primary care visits and same day sick appointments. With this model, providers work autonomously to the top of their licensure while also functioning as part of a team.

The NP is responsible for the assessment and management of various populations of patients, medical and/or surgical, including diagnostic and therapeutic interventions, development of appropriate plans of care, and ongoing evaluation.Does this position require Patient Care?

Essential Functions
-Provides direct care, counseling, and teaching to a designated patient population in the ambulatory, inpatient, operative, and/or procedural setting.
-Performs complete histories and physical examinations.
-Orders, interprets, and evaluates appropriate laboratory and diagnostic tests.
-Develops appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings.
-Orders medications and writes prescriptions according to organizational and regulatory policies and procedures.
-Consistently provides high-quality and timely documentation, including admission and progress notes, procedure notes, operative notes, and discharge summaries.
-Performs bedside procedures as appropriate to the patient population.
