We are seeking a full-time Nurse Practitioner (40 hours per week) to join our Family Medicine practice at 21 Elm Street in South Deerfield, MA. Our hours of operation are Monday–Friday, 8:00am–5:00pm, with a weekly schedule of 36 hours of direct patient care and 4 hours of administrative time, designated on Fridays.
Our South Deerfield office is a close-knit, community-centered practice dedicated to comprehensive primary care for patients of all ages. We provide physical exams, routine screenings, preventive care, and the diagnosis and management of acute and chronic conditions—with a strong focus on prevention and long-term wellness. Primary care is about relationships, and we take pride in truly knowing our patients to deliver exceptional, personalized care. This role will support a patient panel of approximately 1,000 patients.

Job Summary
The Nurse Practitioner (NP) is a licensed provider. The NP is responsible for assessing and managing various patient populations, including those with medical and/or surgical conditions, as well as providing diagnostic and therapeutic interventions, developing appropriate plans of care, and conducting ongoing evaluations.Does this position require Patient Care?

Essential Functions
-Provides direct care, counseling, and education to a designated patient population in ambulatory, inpatient, operative, and/or procedural settings.
-Performs complete histories and physical examinations.
-Orders, interprets, and evaluates appropriate laboratory and diagnostic tests.
-Develops appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings.
-Consistently provides high-quality and timely documentation, including admission and progress notes, procedure notes, operative notes, and discharge summaries.
-Performs bedside procedures as are appropriate to the patient population.
-Orders medications and writes prescriptions in accordance with organizational and regulatory policies and procedures.
