Job Summary The Advanced Practice Provider (APP) cares for our patients within the scope of training and approved the agreement, as outlined by their governing board. Works collaboratively with other providers and staff to ensure efficient patient flow and a high level of patient satisfaction. Responsibilities Perform complete, detailed, and accurate health histories, review patient records, develop comprehensive medical assessments, and order laboratory, radiological and diagnostic studies appropriate for complaint, race, sex, and physical condition of the patient Formulate medical and nursing diagnoses and institute therapy or referrals of patients to the appropriate health care facilities, agencies, and other resources of the community or physician Institute emergency measures and emergency treatment or appropriate stabilization measures in situations such as cardiac arrest, shock, hemorrhage, convulsions, poisoning, and allergic reactions Interpret and analyze patient data to determine patient status, patient management and treatment Provide instructions and guidance regarding health care and health care promotion to patients/family/ significant others Other duties and responsibilities as assigned Qualifications Excellent communicator with staff, patients, and family Professional appearance and attitude ANCC, AANP, or NCCPA Board Certified – (with 1 yr ER/UC experience preferred) Active and current Physician Assistant or Nurse Practitioner licensure in the state of practice, DEA number, and state-controlled substance certificate, as required Able to multi-task and work independently

Nurse Practitioner

POSITION SUMMARY: A licensed and credentialed Nurse Practitioner (NP) provides diagnosis, treatment, consultation, and follow-up under the direction of 1 or more physicians in the clinic. The NP performs age and specialty appropriate physical examinations and preventive health measures within prescribed guidelines. As an expert professional, the Nurse Practitioner is responsible and accountable for the application of the nursing process, supervision of clinical practice, research involvement, consultative services, inter and intradepartmental liaison, education, leadership and patient care management across the health care continuum from preventative, acute care through discharge. Responsibilities including management and peri-procedural care of outpatient cardiac catheterization patients, as well as primary role as Valve Program Coordinator, which includes coordination of Cardiac Valve Clinic (in concert with CT surgery), navigating patients through valve / structural heart workups, and administrative duties involving structural heart registry, follow-up. Under general supervision of a cardiologist, the nurse practitioner would be responsible for providing relevant medical services which may include obtaining complete medical history and performing physical examination, ordering and interpreting laboratory tests and imaging studies; formulating a diagnosis and treatment plan; prescribing and/or dispensing medication; counseling patients and families, and other patient care duties. Expert clinical judgment and ability to be self-directive and demonstrate initiative in an expanded role. KNOWLEDGE AND SKILLS: Successful candidates will be self-directed, demonstrate initiative and professionalism, and exercise expert clinical judgment to function independently in this role. Excellent interpersonal and communication skills in English are required in order to communicate complex medical information to collaborators such as patients and their families, physicians, staff, and community health workers, and willingness to address patients with language or social barriers to healthcare.
