Oncology experience is a plus, and previous Infusion Oncology experience highly preferred. JOB SCOPE: Under general supervision, provides professional nursing care for oncology patients following established standards and practices. Demonstrates knowledge of age specific differences in the patient population including late adolescence, adulthood, middle age, and the elderly. This includes physiological, psychological, and developmental differences unique to each group. Supports and adheres to the US Oncology Compliance Program, to include the Code of Ethics and Business Standards. Responsibilities Assists physician with exams and procedures. Maintains efficient and appropriate patient flow in and out of exam rooms. Assesses patients for changes in condition, including but not limited to abnormal labs, x-rays, scans, drug reactions and initiates appropriate action. Facilitates dose modification by assessing patient, assessing labs, consulting protocol or treatment plan, calculating dose, and consulting physician regarding dosage modifications, problems, and symptom management. Performs venipuncture, accesses implanted ports and maintains central lines catheter devices. Administers all medications/blood products using correct medication/blood product, dosage/amount, route, time and technique. Documents nursing assessments, patient teaching and fluid and medication administration in the patient chart. Communicates all pertinent information and any interventions to the physician. Assures nursing charges are accurate and complete on patient fee slips. Acts as a resource person to the medical assistants. Triages patient phone calls as needed and documents relevant information in the patients chart. Assesses patient/family needs and provides teaching and other instructional material. Demonstrates awareness of community resources available to patients and assists with referrals if necessary. Manages patient medication refill requests from the pharmacy telephone line and records the information in the patient chart. Provides nursing coverage for the satellite offices as needed. Follows OSHA guidelines when handling cytotoxic agents, biohazardous waste and any other material potentially infected with a blood borne pathogen. Maintains patient confidentiality at all times. Participates in cross-training programs to develop skills across multiple areas of the practice and provide support where needed. Qualifications Graduate from an accredited program for professional nursing education, BSN preferred. Dual licensure in Oregon (OR) and Washington (WA) must be obtained within 6 months of employment start date. Chemotherapy Immunotherapy provider card, preferred. Current Basic Life Support (BLS) certification. **Must maintain continuing education requirements. Experience/Training: Minimum (1-3 years) of nursing experience including at least (1) year in oncology preferred. The APP is credentialed by the Credentials Committee and approved by the Professional Board of OHSU and other locations as required to practice in the hospital setting and to perform such procedures as authorized by the Credentials Committee and by the Professional Board. The APP is authorized to prescribe medications according to the state and federal guidelines and laws. This faculty position specializes in caring for adult patients with neurological disorders and is primarily responsible for inpatient clinical care as well as other clinical operations within the Department of Neurological Surgery. The role involves independent patient care, procedural work, care coordination and administrative tasks, and educational contributions. The APP collaborates closely with faculty physicians, APP colleagues, and others in the interdisciplinary team to ensure the highest standards of patient care and service. The APP will support OHSU’s missions including teaching, research, patient care, outreach, and public service. Clinical Care Primarily in an inpatient environment that provides 7-day/week clinical coverage, provide comprehensive care for neurosurgery patients from admission to discharge. Perform administrative patient care for patients who recently discharged and collaborate with outpatient teams members to prepare patients for next steps in their care. Respond promptly to patient/family inquiries (phone, MyChart), collaborating with team members such as nurses, APPs and physicians to resolve clinical concerns. Identify urgent clinical situations and respond autonomously and effectively, including providing situational resuscitation and emergency treatment. Provide scheduled weekend hospital coverage including rounds, patient discharges, and continuity of care duties according to department protocol and rotating assignments. Care Coordination & Communication Coordinate with interdisciplinary teams, including outpatient staff and other APPs, to ensure streamlined, patient-centered care. Collaborate with surgeons, physicians, and external providers to optimize patient outcomes. Triage patient calls, messages, and requests and determine the appropriate plan of care for recently discharged patients. Educate house staff and clinic medical teams on care plans and best practices to improve patient outcomes. Patient and Family Education Contribute to patient and family education, including development and presentation of educational materials. Provide condition-specific education and counseling to patients and families. Use available educational materials and resources to promote patient understanding and self-management. Program Development, Teaching & Quality Improvement Engage in continuous process improvement, adhering to OHSU policies, service standards, and regulatory guidelines. Assesses and manages patients' response to home dialysis training and treatment therapy by following prescribed predetermined protocols. Communicates patient related issues to the physician as needed. Reports adverse patient events at home or in the clinic, as well as equipment and technology related problems to the Clinical Manager/Home Therapy Program Manager, and physician including the correct documentation of such events. Coordinates the transfer or discharge of patients to the hemodialysis or transplant clinic when needed and appropriate. Reviews, transcribes, and enters physician lab orders accurately into Proton,AMI or Ecube medical information systems as appropriate. Follows company policies and government regulations related to the collection, packaging, and delivery of lab specimens and reporting of results. Maintains competency with all emergency operational procedures, and initiates CPR and emergency measures in the event of a cardiac and/or respiratory arrest when the patient is in the dialysis facility. Administers medications as prescribed including following prescribed algorithms (as appropriate), and documents appropriate medical justification if indicated and maintains records on controlled substances as required by law. Documents all relevant data including physician orders, lab results, vital signs, and treatment parameters, and patient status and any treatment related data appropriately, and enters into the appropriate medical information system. Completes all monthly and annual reporting as required by FMS policies and government regulations. Submits CQI information to Clinical Manager or Program Manager for completion of HT QAI Template for inclusion in facility QAPI review process. Participates in all monthly QAPI meetings and all required staff meetings as scheduled. Rotates coverage with other licensed staff to ensure reliable and adequate coverage and participates in 24/7 on-call coverage as assigned. Participate in staff training and orientation of new staff as assigned. Maintains appropriate skill level in the operation of all home dialysis therapy-related equipment, including the ability to troubleshoot equipment problems with the patient via telephone. Directs patients in the appropriate routine maintenance of home dialysis equipment, including water systems procedures as appropriate, immediately reporting any water/dialysate problems to the facility Home Dialysis technical support staff. Coordinates with the FMCNA RTG the ordering and delivery of the patient's initial dialysis supplies; the ordering of the patient's dialysis equipment as appropriate at the initiation of the training program; and the on-going operation and preventative maintenance of all home dialysis equipment through facility Technical support staff, or through the appropriate equipment vendor as required. Transitional Care Unit - RN - Working with Hemodialysis patients to support the possibility of moving into Home Hemodialysis treatments PURPOSE AND SCOPE: The professional registered nurse Outpatient RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. This may include delegation of appropriate tasks to direct patient care staff including but not limited to RNs, LVN/LPNs and Patient Care Technicians. As a member of the End Stage Kidney Disease (ESKD) health care team, this position participates in decision-making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and facility operations. POSITION SUMMARY We seek a dedicated Lead Medication Technician (Med Tech/CMA) to provide compassionate, direct care to our residents, assisting them with medication administration and activities of daily living, following a personalized medication and treatment plan. Monitor residents' responses to medications and report any adverse effects or concerns to the HWD. Maintain accurate records of medication administration and resident care activities. Order and receive medication. Conduct regular audits of medication storage and handling practices to ensure compliance with standards. Regularly review the resident’s medication administration record, centrally-stored record, and pharmacy transaction record, and report any errors to the HWD. Provide education and support to residents and their families regarding medication management and health conditions. Assist the Resident Care Coordinator with scheduling as needed. Prioritize independence and provide emotional support to the residents by providing continuous engagement and companionship. According to the individual care plan, assist the residents with the daily administration of prescribed medication, including self-administration. Requirements CANDIDATE QUALIFICATIONS Education and Certifications: High School Diploma or equivalent. Must have a valid state medication administration license (such as CNA/CMA/CMT/QMAP) or meet the state requirements for medication administration in assisted living. Current First Aid and CPR license or ability to obtain. Experience, Competencies, and Skills: At least two (2) years of experience administering medications in a professional caregiving setting. Experience with memory care is preferred. Excellent attention to detail and high integrity. Strong leadership and communication skills, and a teamwork mindset. Coordinates discharge planning and prepares discharge summaries with patient instructions and thoroughly reports patient care needs, progress and goals when transferring care. Demonstrates compliance with agency operations, Option Care’s policies and procedures, professional standards, local, state, federal regulations/guidelines, and accreditation standards. Maximizes work efficiency through the use of computers and other automation technologies to validate plan of treatment orders, communicates patient care provided, and follows assigned visit schedule.
