Adheres to requirements of both CHPC and Hospice Buffalo policies and procedures that are both patient focused (clinical) and organizational focused (administrative). Adheres to HIPPA regulations and policies. Maintains the security of patient electronic/written records. Elicited no founded patient/family complaints or unusual occurrences (i.e. reportable incidents). Displays an ability to establish relationships and maintain/collaborate positive rapport with other access and hospice clinical staff and hospice support staff, hospital staff, and the medical community. Demonstrates a bias for action, acts on what is important, is proactive and creative, possess a "can do" attitude, has an ability to work under pressure and keep difficult situations in prospective. Participates in the weekend, holiday schedule. Qualifications for an Admissions Liaison Registered Nurse: Education / Certification Graduate of an accredited school of nursing with current RN licensure in the State of New York. Certification in Hospice and Palliative Care Nursing preferred. Valid Driver's License, free of serious violations, and vehicle required for use when on duty or on-call. Experience 2 years of nursing practice as a professional nurse is preferred.
