Essential Functions: Utilizes the nursing process: assessment, planning, intervention, implementation, and evaluation and formulating nursing interventions directed towards development of an individualized plan of care and treatment modalities. Identifies risk levels, precautions, and expected outcomes individualized to the client to include discharge planning; prescribes interventions to attain expected outcomes; evaluates the effectiveness of interventions in relation to expected outcomes; conducts ongoing assessment of clients from admission to discharge; initiates medical/psychiatric crisis intervention to stabilize and ensure the well-being of the patient's conditions. Demonstrates knowledge of psychiatric disorders and appropriate nursing interventions; implements nursing interventions identified in the plan of care that is individualized to the patient needs. Applies appropriate interventions to progress the patient towards wellness and addresses maintenance of wellness. Administers medications as prescribed safely; provides education on prescribed medications; recognizes signs and symptoms of adverse drug reactions. Demonstrates professionalism, accountability, and responsibility for decision making, quality nursing care, and knowledge base of psychiatric nursing care. Reviews, revises, and updates medical records within facility guidelines and timeframes. Transcribes and implements physician's orders according to facility policy; documents care, progress, response to intervention in medical record in an accurate, thorough and concise manner. Follows up on client care responsibilities, communicates incomplete assignments, and current client needs to staff to assure continuity of care. Implements staff assignments and delegation of duties using appropriate judgment and knowledge; Demonstrates positive willingness to assist staff and patients; functions as an advocate for patient needs/safety Utilizes nursing knowledge to successfully meet the needs of the facility, patient population, and staff members utilizes nursing resources to enhance evidence-based practice Float among various clinical services, where qualified and competent and performs other duties and projects as assigned.

This position makes a significant contribution to MCS’s mission by conducting monitoring visits in Texas nursing facilities. The Nurse Quality Monitor III utilizes expertise in the area of nursing to assess conditions that could place facility residents’ health, safety, and welfare at risk and assists providers in improving systems. This position must be able to apply the complex knowledge of nursing practice as well as the rules outlined in the Texas Nursing Practice Act. The Nurse III will be familiar with local, state, and federal laws and regulations relevant to long-term care. The Quality Monitor Nurse (QMN) III works under limited supervision, with considerable latitude for the use of initiative and independent judgment. Performs highly complex (senior-level) nursing work. The Quality Monitor Nurse utilizes expertise in the area of nursing to assess conditions that could place facility residents’ health, safety, and welfare at risk and assist providers in improving systems. Work may include serving as a resource on nursing practices and education programs by performing consultative and administrative work. May serve as a mentor providing direction to others. Essential Job Functions Attends work on a regular and predictable schedule in accordance with agency leave policy and performs other duties as assigned. Monitors and evaluates facility systems through observations, interviews, and record reviews identifying facility strengths and weakness making recommendations on opportunities for improvement. Reviews facilities policy/procedures and performance reports to determine if the facility's systems are consistent with evidence-based best practice. Maintains current knowledge in assigned areas by attending unit meetings, seminars, or other training and by continually reviewing program policies, procedures, and any changes. May conduct literature reviews and analyze complex information to update nursing-related evidence-based best practices technical assistance material. (10%) Develops and provides in-service education and other training to facility-licensed and non-licensed staff regarding evidence-based best practices. Develops data collection tools, identifies trends, risk areas, and evaluates reliance of research. Coordinates with employees to collect data on the quality of Nursing services; analyzes data collected and submits monthly report to Performance Improvement Director. Oversees and adheres to safety regulations of facility policies and procedures and external regulatory and credentialing bodies; participates as a member of the facility wide safety team. Provides support and acts as a resource for development and implementation of Infection Control, standard precautions policies and procedures. Supports and coordinates patient safety issues in conjunction with Hospital Administrator; analyzes, reviews and follows up on incident reports related to nursing/patient safety issues in conjunction with Risk Management. Ensures staff compliance with patient care, documentation, hospital/unit policies and procedures, protocols, and guidelines; Ensures appropriate nursing staff for each unit to meet patient needs. Directs appropriate training activities for all nursing employees as well as assurance of proper current licensure, continuing education, CPR, and an Oceans Healthcare approved behavioral health de-escalation program; participates in orientation of new and current nursing staff. The nursing staff on GTSCU are trained to care for patients at the end of life as well as those who will be discharged home or to other facilities.
