The Day Center CNA ( Certified Nurse Assistant) works at our Adult Day Health Center assisting participants with ADLs (activities of daily living) and providing both restorative and supportive care. Document care provided in accordance with established policies, guidelines, and applicable regulations. Carry out, reports and documents care given in an effective, timely manner as observed by the RN and through periodic record reviews. Assist participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, fills oxygen and provides other aides as assigned or needed. Monitor participants health status such as vital signs, weights, skin condition, appetite and changes in functional ability and report pertinent information to clinical supervisor in a timely manner. Collaborate with multidisciplinary teams in care planning for participants. Monitor appetite and look for changes in functional ability and skin condition. Monitor/assist participants in a courteous manner with meals and helps with feeding, as necessary. Report any variance, accident, or unusual occurrence to the appropriate personnel. Assist with loading and unloading of participants from vehicles. Assist the transportation department with participants’ take-home supplies. Serve as a bus attendant on company vehicles while InnovAge drivers transport participants to/from center or medical appointments supporting participants while in transit and accompanying participants in and out of facilities. May rotate into In-Home Services as a Homecare CNA. as deemed necessary by the Center Manager. Creates and participates in group(s) and one-on-one activities in accordance with daily activity calendar when needed. Attend in-service and continuing education programs as scheduled and necessary. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Administrator.
