Performs duties demonstrating a rehabilitation knowledge base. Using hospital and regulatory standards of care for rehabilitation, the nurse demonstrates sound clinical judgment in the development, implementation and evaluation of the nursing aspects of the trans-disciplinary care plan. In conjunction with the patient/family, the nurse develops an individualized patient care plan and determines interventions based on assessment/reassessment findings. The nurse evaluates patient outcomes on an ongoing basis, establishes priorities for patient care services as evidenced by modification of care plan, if necessary, and demonstrates the ability to accurately document patient care activities in the medical record. Utilizing a case management approach which initiates discharge planning on admission, the nurse plans, and guides, directs, coordinates and collaboratively incorporates the overall aspects of patient care across the continuum by effectively communicating with physicians, staff and other disciplines. Safely and competently administers pharmacotherapeutics adhering to medication policies, procedures, practices and standards, and monitors the resultant outcomes based upon the patient’s response to treatment and its effect on the rehabilitation plan of care. Assesses and responds appropriately to ensure patient safety based on the physical environment, equipment and the patient’s behavior. Demonstrates the ability to provide care in emergency situations and follows established emergency procedures and practices. Communicates and follows the organizational chain of command for notification of patient care/service issues, when appropriate. Maintains knowledge of, and demonstrates the ability to identify therapeutic rehabilitation patient care issues/problems and determines measures and interventions to promote, maintain and/or restore health for the adolescent through geriatric rehabilitation patient population as described in the Rehabilitation policies/procedures including the skills competency check list. The nurse also provides appropriate care through the healthcare continuum including end of life. Assures that the rights of all patients are respected and maintained by allowing for privacy and dignity in the provision of care. Determines the presence and type of Advance Directive, if applicable. The nurse ensures patient and/or family involvement in the decision-making process as it relates to patient care, end of life and ethical issues. The nurse is aware of the reporting responsibilities and reports suspected cases of abuse or neglect relative to the adolescent through geriatric patient population as appropriate. Identifies patient/family educational needs; initiates and provides individualized teaching which considers the functional/developmental age and the needs of the adolescent through geriatric rehabilitation patient population. The nurse evaluates and documents the effectiveness of the patient/family teaching through assessment of their comprehension of the instruction and demonstration of appropriate responses. A Certified Nursing Assistant (CNA) is responsible for providing routine nursing care in accordance with established policies and procedures and as may be directed by the Charge Nurse, RN Supervisor, Director of Nurses or Administrator, to assure that the highest degree of quality resident care can be maintained at all times. Assist residents in dressing as necessary. • Assist in preparing residents for meals (taking to/from dining room, serving trays, placing bibs, assisting in feeding or cutting food, removal of trays, supervision in dining room, etc.). • Serve nourishment in accordance with established facility procedures. • Feed residents who cannot feed themselves. • Assure that resident’s food is accessible and self-help devices are available as needed. • Prepare residents for therapy, activities, physician visits, religious services, etc., to include transporting residents to/from such activities. • Keep incontinent residents clean and dry as possible at all times. Change bed linens, diapers and clothing as often as necessary. • Do not permit residents to smoke in bed. • Assure that the nurse’s call system is attached to the bed and within easy reach at all times when residents are bedfast. • Answer residents’ call lights promptly. • Check for food in residents’ room. Ensure that food is kept in a sealed container. • Report all complaints to the Charge Nurse. • Report any resident abuse immediately (ie., harsh/abusive language, unnecessary roughness, etc.) • Report any medication found in the resident’s room. Take to the Charge Nurse immediately. • Report all hazardous conditions and faulty equipment to the Charge Nurse immediately. • Report any bruise, skin tears, incidents or accidents to the Charge Nurse immediately. • Attend in-service educational programs, on the job training programs and meetings as directed. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. Keep dentures clean and soaked with Efferdent after dinner. • Record resident’s food and nourishment intake as directed. • Perform after meal care as required – cleaning resident’s hands, face, clothing, dentures, etc. • Assist residents to/from bathroom. Offer bedfast residents bedpans and urinals. Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident’s hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. The Hospice Team Nurse provides professional nursing care to patients by utilizing all elements of nursing process. 2. . Assesses and evaluates patient’s status 3. Initiates the plan of care and makes necessary revisions as patient status and needs change. 4. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 5. This position will be TUESDAY-SATURDAY 12PM-8PM. 6. This position will be PRN. 7. This position will be working in different service areas such as the following - Alameda, Contra Costa, Napa, Santa Rosa, Solano and Sonoma. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload. Communication 1. The Hospice Team Nurse completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. 2. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. 3. Communicates with community health related persons to coordinate the care plan. POSITION QUALIFICATIONS 1. Graduate of a National League of Nursing accredited school of nursing. 2. Current licensure in State of CA and CPR certification. 3. Registered nurses / Licensed Vocational nurse shall have a minimum of one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing from a program accredited by the National League of Nursing and a current RN / LVN license. Minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Management experience not required. Responsible for supervising hospice aides. 5. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. 6. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. 7. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Provides clinical data and observations to contribute to the nursing plan of care CLINICAL • Admits, transfers, and discharges residents in accordance with policy and procedure • Orients new residents/families as necessary • Gives/receives nursing reports upon beginning and ending of assigned shift. Reviews daily patient report and assignments with Nursing Assistants upon the beginning of assigned shift • Assures that the unit area is maintained in a clean and safe manner for resident comfort and convenience. • Conducts daily resident rounds to observe the resident’s physical and emotional status • Collects clinical data and reports significant clinical findings according to policy • Performs manual and technical skills according to the scope of practice under the direction of a Registered Nurse • Records care information accurately, timely and concisely. Completes all required documentation including resident observations, interventions, and patient response(s) in the medical record in accordance with policy • Assists in developing, reviewing, revising, and updating resident Plans of Care as indicated. Contributes to the evaluation of the patient’s progress towards specific goals and the adjustment of the nursing plan of care as necessary • Receives and transcribes orders accurately from attending/alternate physician. • Assists in making rounds with the attending physician, as necessary. • Prepares/administers medications as ordered by the physician and within the legal scope of practice. • Completes medical treatments as indicated and ordered by the physician. • Evaluates response and report adverse reactions to medication or treatments in accordance with the policy. • Assures that narcotic and controlled drugs are secure, inventory counts are accurate and narcotic register documentation is timely and accurate • Orders prescribed medications, supplies and equipment as established by the facility’s policies and procedures. • Requisitions and arranges for diagnostic and therapeutic services as ordered by the physician. • Notifies the attending/alternate physician, as well as the resident’s legal guardian/representative, when the resident becomes critically ill, injured, etc., or has a significant change in condition as outlined within established policies. • Notifies the attending/alternate physician of resident incidents/unusual occurrences. Completes Accident/Incident Report forms as indicated and documents relevant information in the resident’s medical record as outlined in established policies and procedures. Initiate investigation of accidents and unusual occurrences • Informs RN Supervisor or RN in charge of the facility of significant changes in resident condition, unusual occurrences, incidents, or accidents as indicated and per facility policy and procedure • Attend various facility meetings (care conferences, stand-up meetings, staff meetings, Medicare meetings, discharge plan meetings, etc.) as required. • Reports allegations of abuse or unusual occurrence per State and Federal Regulations and facility policy. Assists with a gathering of information relative to the event. • Initiates emergency procedures (including basic CPR) and provide emergency patient treatments • Participates in the facility’s infection control program for monitoring communicable and/or infectious disease among residents and staff. • Participates in procedures for reporting hazardous conditions or equipment. • Assists in orientation of newly assigned personnel as requested. • Assumes Charge responsibility following orientation period as requested. • Performs other position-related duties as assigned. • Supervise CNAs SUPERVISION • Make resident rounds to ensure appropriate care is being rendered, identifying, and making corrections as necessary • Meet with nursing personnel to assist in identifying and correcting problem areas and/or the improvement of resident care • Assist the Charge Nurse in revision and distribution of nursing assignments as needed • Assist in the supervision and direction of nursing personnel • Participate in disciplinary action and evaluation of nursing personnel CONSUMER SERVICE • Presents a professional image to consumers through dress, behavior, and speech. • Treats residents/family members with dignity and respect • Responds to Resident and family concern(s) and assists in resolving grievances. A licensed professional nurse directly under the supervision of the Director of Nursing Services. The Charge Nurse is responsible for coordinating daily administrative duties of an assigned nursing unit including schedules, nursing assignments, delegation of resident care and supervision of nursing staff. Report missing ID bands to admission coordinator immediately. • Make beds, clean bedside and overbed tables. • Serve drinking water as required. Assure that water pitchers are cleaned, filled with fresh drinking water and within the resident’s reach. Pass water pitcher as per established policy and procedure. • Dress residents neatly and in their own clothing. Assist residents in dressing as necessary. • Assist in preparing residents for meals (taking to/from dining room, serving trays, placing bibs, assisting in feeding or cutting food, removal of trays, supervision in dining room, etc.). • Serve nourishment in accordance with established facility procedures. • Feed residents who cannot feed themselves. • Assure that resident’s food is accessible and self-help devices are available as needed. • Prepare residents for therapy, activities, physician visits, religious services, etc., to include transporting residents to/from such activities. • Keep incontinent residents clean and dry as possible at all times. Change bed linens, diapers and clothing as often as necessary. • Do not permit residents to smoke in bed. • Assure that the nurse’s call system is attached to the bed and within easy reach at all times when residents are bedfast. • Answer residents’ call lights promptly. • Check for food in residents’ room. Ensure that food is kept in a sealed container. • Report all complaints to the Charge Nurse. • Report any resident abuse immediately (ie., harsh/abusive language, unnecessary roughness, etc.) • Report any medication found in the resident’s room. Take to the Charge Nurse immediately. • Report all hazardous conditions and faulty equipment to the Charge Nurse immediately. • Report any bruise, skin tears, incidents or accidents to the Charge Nurse immediately. • Attend in-service educational programs, on the job training programs and meetings as directed. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. CLINICAL • Wash hands before and after caring for each resident. • Place residents in correct and comfortable position. Head of bed elevated at 30 for all residents on enteral feeding. • Assist residents with dental and mouth care. Clean dentures after each meal. Keep dentures clean and soaked with Efferdent after dinner. • Record resident’s food and nourishment intake as directed. • Perform after meal care as required – cleaning resident’s hands, face, clothing, dentures, etc. • Assist residents to/from bathroom. Offer bedfast residents bedpans and urinals. Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident’s hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator.
