Supervising care treatment and services of patient care provided by LPNs, CNAs and other staff on assigned unit and shift. Developing, implementing, and evaluating an individual plan of care for each client. Assessment and re-assessment of patients. Communicating appropriate information with the interdisciplinary team. Reporting client concerns and changes in condition to supervisor and/or physician in a timely manner. Roles and Responsibilities: · Provides and coordinates care by assessing physical and behavioral health needs of client, develops and implements nursing care plans, maintains medical records and educates clients and their families about various physical and behavioral health conditions while ensuring client safety at all times. · Assesses client’s status on an ongoing basis and pertinent information gathered is documented. · Ensures nursing treatment plans are specific to assessed needs and initial treatment plan is completed upon admission. · Takes physician’s orders (verbal or via telephone) accurately and documents appropriately; transcribes and reviews physician’s orders. Ensures physician’s orders are legible if taken verbally and transcribed appropriately per policy and procedure. · Accurately administers medications per ordered note and documents accurate count of controlled substance. Adjusts medications under provider’s supervision as needed; monitors lab results to validate medication compliance. · Notifies physician of significant changes in status such as difficulties with detox, medical emergency or change in medical status regarding response to medications, etc. · Supervises self-administration of medications using safety initiative of using two means of client identification: documents and reports effects of scheduled and PRN medications. · Maintains accountability of administration, recording, control and security of medications, including narcotics. · Completes admission and discharge processes efficiently and accurately as directed. · Confers regularly with treatment team (e.g., nurses, therapists, program managers, physician) regarding client care. Attends and participates in the unit activities such as therapy groups, family meetings as appropriate, staffing, unit meetings, etc. · Provides education/explanation to clients, families, and co-workers regarding medication usage/maintenance, common side effects, client's rights, as well as other topics necessary for optimal client care. · Monitors client acuity, census, staffing needs, and makes arrangements for coverage as needed, following established guidelines. · Assures thorough, accurate documentation is completed for all clients on each shift and client assignment sheets are completed daily and tasks are carried out as assigned. · Gives thorough report to oncoming staff, supervisors, and physician. · Provides coverage in emergency services as needed. EDUCATION/EXPERIENCE/SKILL REQUIREMENTS: · Graduation from an approved/accredited school of nursing. · One or more years’ nursing experience within clinical area of service provided by facility preferred. · Experience in behavioral health/psychiatric/addiction recovery environment preferred. LICENSES/DESIGNATIONS/CERTIFICATIONS: · Valid current RN license from the state or from a compact state, where applicable. · Certified in Psychiatric Nursing preferred. Overview The General Internal Medicine (GIM) Clinic provides preventative, acute and chronic care to all adults, 18 years and older. The GIM Clinic is a fast-paced clinic, with over 26,000 visits annually. The General Internal Medicine team is a growing team with highly sought-after clinicians and care team members to support all patients needs. This care team consists of a professional community, working to ensure patient care needs are met: Physicians, Nurse Practitioners, Physician Assistants providers, nurses, social workers, medical assistants (MA), nursing assistants (LNA), Diabetic Educator, Behavioral Health Clinicians, Dietitian, and secretaries. The leadership team is very much relationship based, and promotes teamwork, collaboration, effective communication, safe and quality patient care, and a positive environment for all staff and patients. The clinic provides comprehensive care to patients while open Monday through Friday 0800-1700 . The role of the nurse in the GIM clinic is vital to symptom management, triage, patient education, in clinic procedures, as well as collaborative and independent nurse visits. The nursing team is structured to provide support to both patients in clinic and via phone. This position does allow for flexible hours depending on candidate and operational needs. The ideal candidate would be able to provide clinic support four days a week, 10-hour shifts, OR five days a week, 8-hour shifts. We are seeking a dedicated and compassionate Pediatric Intensive Care Unit (PICU) nurse to join our team. This role involves providing specialized, critical care to infants, children, and adolescents facing life-threatening conditions. PT Schedule is 2x12hr shifts rotating 7a-7p, 7p-7a Responsibilities The Clinical Nurse is an engaged and credentialed member of the Professional Nursing Organization and is responsible for autonomous practice directed by the professional tenets of practitioner, leader/decision maker, scientist and transferor. The Clinical Nurse is responsible for utilizing the nursing process to provide evidence-based care and to continuously monitor and evaluate practice to ensure safe passage of patients that is in the best interest of populations served. Practitioner Utilizes the nursing process to assess, diagnose, identify outcomes, plan, implement and evaluate an individualized plan of care. Utilizes critical thinking and the nursing process to anticipate and recognize changes in patient status, taking action to modify the plan of care or to elevate to the care team as necessary. Practices in accordance with the ANA Code of Ethics to advocate for patients, uphold their autonomy in decision-making, ensure informed consent and assist patients in families in expressing self-determination. Actively seeks feedback and acts to improve performance. Engages in the governance of practice. Leader/Decision Maker Manages interpersonal relationships for self and with others. Mentors colleagues for the advancement of nursing practice and the profession. Assumes authority and accountability for the nursing care of patients while appropriately delegating elements of care to others members of the care delivery team in accordance with laws, regulations and policies and procedures. Precepts the student nurse, nurse extern, nurse resident, experienced clinical nurse and other members of the healthcare team. Performs other duties as required or assigned. Qualifications Graduate from an accredited Nursing Program required. Bachelor of Science Degree in Nursing (BSN) preferred. Required Licensure/Certifications Licensed Registered Nurse with New Hampshire eligibility. Basic Life Support (BLS) Certificate required upon hire PALS Certificate required within 6 months of hire date.
