Rapidly growing service line adding provider coverage to meet the growing need in the community. the NP/PA will evaluate, diagnose and treat patients following written guidelines approved by the Department of Medicine. Compensation Pay Range: $107,000.00 - $224,123.00 ESSENTIAL FUNCTIONS Essential functions are those tasks, duties and responsibilities that comprise the means of accomplishing the job's purpose and objectives. Following are the essential functions of the job. 1 – Provide telehealth services to patients and community partners to operationalize COVID testing and/or discharge follow ups after ED/Hospitalization for COVID symptoms 2 – Telehealth evaluations and coordination for community based testing and incoming phone requests from SNFs, ALFs and Psychiatric Hospitals 3 – Provider will complete Provider to Provider screening and place orders for testing for symptomatic home-bound and at-risk populations Documentation in SSH MIH Epic Code for provider to provider consultation Provider will coordinate with Registration to assure patient information is updated in Epic Provider will coordinate with EMS Dispatch to print lab labels and dispatch MIH/EMS to collect specimens 4 – Telehealth follow up with discharged patients Patients will be discharged from the ED or Acute Inpatient per algorithm At time of discharge, a Home Monitoring Telehealth follow up visit will be ordered Provider will review Home Monitoring Telehealth Report Provider will outreach to each patient on the report and conduct a Telehealth follow-up visit 5 – Documentation in SSH MIH Epic Code for Telehealth visits (telephone or video) 6 – Provider will determine next course of action based on Telehealth visit Patient discharged from program – need to collect device Patient remains in MIH Telehealth program – schedule Telehealth visit for next day (remains on list) Patient required MIH Paramedic visit – Provider to place order and coordinate with MIH Administration Patient follows ups with PCP – Provider to outreach to PCP and coordinate PCP to follow patient Patient required VNA services – Provider to place order and coordinate with VNA Patient requires Medical Control oversight – Provider to outreach to MIH Medical Control 7 – Telehealth and consultation with MIH paramedics Patients will be referred to program by Providers/Staff MIH paramedic will be scheduled for home visit by MIH admin MIH paramedic will complete evaluation per protocol 8 – If additional consultation is required, MIH paramedic will outreach to MIH Telehealth APC for Telehealth visit with Provider. Provider will determine next course of action based on Telehealth visit: Patient discharged from program – need to collect device Patient remains in MIH Telehealth program – schedule Telehealth visit for next day (remains on list) Patient requires second MIH paramedic visit – provider to coordinate with MIH Administration Patient follows ups with PCP – Provider to outreach to PCP and coordinate PCP to follow patient Patient required VNA services – Provider to place order and coordinate with VNA Patient requires Medical Control oversight – Provider to outreach to MIH Medical Control Technology and Learning a - Participates in continued learning and possess a willingness and ability to learn and utilize new technology and procedures that continue to develop in their role and throughout the organization. b - Embraces technological advances that allow us to communicate information effectively and efficiently based on role.
