Position Summary: The Physician Assistant, Family Nurse Practitioner (PA, FNP) shall be committed to contributing to the mission of the Yukon-Kuskokwim Health Corporation (YKHC), Working together to Achieve Excellent Health." Care for acute and chronically ill adults and children. Provide care to Level IV Trauma Center/ER patients. Diabetes Program Nurse Practitioner/Physician Assistant - Sign on Bonus, Relocation, and Retention Bonus Eligible

Position summary: This is a grant funded position. Diabetes Program Nurse Practitioner is primarily responsible for providing medical management, care and education for persons diagnosed with diabetes and related chronic conditions, as well as prevention and wellness. This position will also assist with ongoing program planning, development and implementation of diabetes-related chronic disease prevention and treatment projects Conduct regular trips to village clinics to provide patient care.
